阿芬太尼对单肺通气患者肿瘤坏死因子-α与白细胞介素-6的影响 被引量:5

Effects of alfentanil on tumor necrosis factor-α and interleukin-6 in patients undergoing one lung ventilation

摘要 目的评价阿芬太尼对单肺通气(OLV)患者围术期肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)及其mRNA表达的影响。方法择期拟行肺叶切除术肺癌患者24例随机分为2组:芬太尼组和阿芬太尼组,每组12例。麻醉诱导后,2组均静脉注射咪唑安定0.03 mg.kg-1、吸入8%七氟醚、2组分别给予芬太尼3μg.kg-1和阿芬太尼18μg.kg-1。麻醉维持:间断静脉注射维库溴铵0.04~0.08 mg.kg-1,芬太尼组0.05~0.1 mg,阿芬太尼组0.3~0.6mg。2组均在麻醉诱导后切皮前(T0)、单肺通气60 min(T1)、膨肺后60 min(T2)采血样,测定TNF-α、IL-6浓度及其mRNA的表达。结果 TNF-α、IL-6浓度及其mRNA表达,2组于T1、T2明显上升(P<0.05);与芬太尼组比较,阿芬太尼组这3项指标于T1、T2均明显降低(P<0.05)。结论 OLV患者用阿芬太尼可有效地抑制机体促炎细胞因子的生成和释放。 Objective To investigate the effects of alfentanil on TNF - α, interleukin -6 and their mRNA in patients undergoing one lung venti- lation (OLV). Methods Twenty - four patients with lung cancer resec- tion were randomly divided into two groups ( n = 12 each) : group I fenta- nyl and group H alfentanil. Anesthesia was induced with midazolam 0. 03 mg· kg^-1, fentanyl 3 μg ·kg^-1(or alfentanil 18 μg ·kg^-1) and 8% sevoflurane, and maintained with intermittent iv boluses of vecuronium 0. 04 -0. 08 mg · kg^-1 , fentanyl 0. 05 -0. 10 mg (or alfentanil 0.3 - 0.6 mg) . Blood sample were taken before skin incision (TO), 60 min after OLV ( T 1 ), 60 rain after lungs were inflated ( T 2 ) for determina- tion of plasma TNF - α, IL - 6 concentration and expression of their mR- NA. Results The levels of TNF-α, IL- 6 and their mRNA were all significantly increased at T 1, T 2 in both groups ( P 〈 0. 05 ). The levels of TNF-α, IL- 6 and their mRNA were lower in group alfentanil than those in group fentanyl at T 1 and T 2 ( P 〈 0. 05 ). Conclusion Alfen- tanil is effective in inhibiting the production of pro - inflammatory cyto- kine for patients undergoing OLV.
