摘要
目的总结腹腔镜下袖状胃切除术的临床应用要点。方法回顾性分析2010年7月至2012年6月上海第二军医大学长海医院普一科89例行腹腔镜下袖状胃切除术的临床资料,手术由同组医师完成,随访时间1~23个月。结果全部手术均经腹腔镜完成手术,无中转开腹,无术中并发症发生,平均手术时间(119.5±16.8)min,平均术中出血量(152.6±51.7)ml。术后患者额外体重减轻百分率(EWL%)呈上升趋势,术后1周、1个月、3个月、6个月额外体重减轻百分率平均分别为11.68%、32.33%、55.83%、74.55%。结论腹腔镜下袖状胃切除术是安全有效的手术,合理选择手术适应证及规范化手术操作及术后管理可以获得良好的临床效果。
Objective To Summarize the key points of laparoscopic sleeve gastrectomy(LSG).Methods The clinical data of 89 patients who had undergone LSG surgery from July 2010 to June 2012 at the Department of Minimally Invasive Surgery,Changhai Hospital were analyzed retrospectively.The operation was performed by the same surgical team,and the patients were followed up for 1-23 months.Results The operations were performed under laparoscopy,with no conversion and operativecomplication.The average operative time was(119.5±16.8) min and the mean intraoperative blood loss was(152.6±51.7) ml.During a 6-month follow-up,weight of the patients,waist circumference and body mass index(BMI) decreased,compared with those before operation.The percentage of weight loss at 1 week,1 month,3 months and 6 months after surgery was 11.68%,32.33%,55.83%,74.55% respectively.Conclusions LSG is a safe and feasible surgical approach.Reasonable choice and operative indications,and postoperative management could help to achievesatisfactory clinical outcome.
