摘要
目的利用脑电超慢涨落图技术(ET)研究脑卒中抑郁症患者治疗前后脑内多种神经递质的变化和脑功能状态的改变。方法采用脑电超慢涨落分析仪,对脑卒中抑郁症患者用药前及用药后2、4、6周各检测1次脑电超慢涨落图,分析抑郁症患者治疗前后神经递质及脑功能动态变化情况。结果脑卒中抑郁症患者治疗前脑内神经递质测值与正常值对照比较,其s1[γ-氨基丁酸(GABA)]、s2[谷氨酸(Glu)]、s3[乙酰胆碱受体(AchR)]、s13[深抑制递质(INH)]测值均高于正常值,其中Glu含量显著高于正常值(P<0.05)。而s6[强兴奋递质(Exc)]、s4[5-羟色胺(5-HT)]、s5[乙酰胆碱(Ach)]、s7[去甲肾上腺素(NE)]、sll[多巴胺(DA)]测值均低于正常值,其中NE、DA含量显著低于正常值P<0.05);治疗前与治疗后2、4、6周各次测值比较,其中GABA治疗后2,4,6周每次测值与治疗前测值,Glu治疗后2、4、6周每次测值与治疗前测值比较均降低(P<0.05)。而5-HT、Ach、NE、DA、Exc经治疗后均明显升高(P<0.05)。HAMD、SDS量表评分比较差异有统计学意义(P<0.05);脑功能状态治疗前后对照比较,也有明显好转。结论脑卒中抑郁症的发生可能与脑内神经递质GABA、Glu、5-HT、Ach、NE、DA、INH物质、EXC物质和脑功能失调有关。
Objective To observe the changes of neurutransmitters in brain and brain function before and after drug treatment in patients with post-stroke depression by means of encephaloflugram technology (ET). Methods Encephaloflugram technoIogy(ET) was used to observe the changes of in brain and brain function before and after drug treatment in patients with post-stroke depression before treatment and at 2,4, 6 weeks after treatment. Results The neurotransmitters levels in brain including GABA, Glu, AchR and INH were higher than those of normal value,in which the Glu levels were significantly higher than those of normal value ( P 〈 0.05 ). However, the other neurotransmitters including 5-HT, NE, DA, AchR and Exc were lower than those of normal controls, in which the NE and DA levels were significantly lower than those of normal controls ( P 〈 0.05 ). The levels of GABA and Glu at 2,4,6 weeks after treatment were significantly lower than those before treatment( P 〈 0.05 ), however, the levels of 5-HT, Ach, NE, DA, Exc were significantly increased after treatment, as compared with those before treatment( P 〈 0.05 ). There was a significant difference in the score between HAMD rating scale and SDS rating scale ( P 〈 0. 05 ). The patient ' s brain function was obviously improved after treatment. Conclusion The pathogenesis of post-stroke depression may be correlated with the neurotransmitters disorder in brain including GABA, Glu ,5-HT, Ach, NE, DA, INH and EXC, and with the disorder of brain function.
