改良大脑半球切除术治疗25例儿童半球病损致难治性癫痫的疗效分析 被引量:3

摘要 目的探讨改良大脑半球切除术治疗儿童难治性癫痫的临床疗效。方法回顾性分析2009年1月至2011年6月采用改良大脑半球切除术治疗25例由一侧大脑半球病损所致儿童难治性癫痫患者的临床资料、手术结果和随访结果。结果按Engel手术分级标准,25例手术患儿中23例获得Ⅰ级效果,2例为Ⅱ级。所有患儿的神经功能缺失症状在术后无明显加重。术后脑电图随访显示,23例痫样放电消失,2例得到部分改善。术后MRI显示健侧大脑半球向对侧生长,同侧硬膜下腔明显缩小。结论对一侧半球广泛性病损所致的儿童难治性癫痫患者行改良大脑半球切除术,可使癫痫发作得到良好控制。 Objective To explore the clinical efficacy of hemispherectomy for pediatric intractable epilepsy.Methods Hemispherectomy was conducted in 25 pediatric patients with intractable epilepsy and unilateral extensive hemispheric lesions between 2005-2010.A retrospective analysis of the clinical data,surgical outcomes and follow-up results of these pediatric patients were performed.Results The seizures of all the patients were effectively controlled.Twenty-three patients achieved Engel class Ⅰ outcome,and 2 patients achieved Engel class Ⅱ outcome.No significant exacerbation of neurological deficits was observed in all the patients.The postoperative EEG was normal in 23 patients,and was significantly improved in 2 patients.The postoperative MRI showed the contralateral hemisphere enlarged and extended to ipsilateral hemisphere.Conclusion For the pediatric intractable epilepsy induced by unilateral extensive hemispheric lesions,hemispherectomy shows good surgical outcomes.
