摘要
目的观察急性心肌梗死患者血浆B型脑钠肽水平与心功能、QTc间期的关系。方法 88例急性心肌梗死患者根据Killip心功能分级标准分为I级32例,Ⅱ级26例,Ⅲ级18例,Ⅳ级12例。根据LVEF水平分为≥50%,41%~49%,≤40%三组,并进行随访,分为发生心血管事件及未发生心血管事件两组,分别检测各组BNP、QTc,并进行统计学分析。结果随着心功能分级的增加,AMI患者血清BNP水平逐渐升高,随着LVEF的降低,AMI患者血清BNP水平逐渐升高,QTc间期逐渐延长,且BNP水平与QTc呈正相关,与LVEF呈负相关,随访6~12个月,其中发生心血管事件28例,未发生心血管事件60例,发生心血管事件的AMI患者血清BNP水平明显高于未发生心血管事件的AMI患者的BNP水平,而QTc间期长于未发生心血管事件的AMI患者的QTc间期(P<0.05)。结论 AMI患者血清BNP水平与QTc呈正相关。通过检测血浆BNP浓度可作为AMI疾病病情监测的一个快速、简便、有效的检验指标。
Objective To observe the acute myocardial infarction in patients with plasma B-type brain natriuretic peptide levels and cardiac function of QTc interval relationship. Methods A total of 88 cases of acute myocardial infarction, Killip cardiac function grading standards, divided into Class I 32 cases of 26 cases, grade Ⅱ, grade Ⅲ 18 cases, 12 cases of grade IV. Divided into LVEF level ≥50% ,41%-49% and ≤40% three groups, and follow-up was divided into cardiovascular events and cardiovascular events did not occur two groups were detected in each group BNP, QTc, and conducted statistical Analysis. Results With the increase of cardiac function classification, AMI in patients with BNP levels gradually increased, LVEF reduction in AMI serum BNP levels gradually increased, QTc interval gradually extended, and BNP levels and QTc was positively correlated with LVEF was negatively correlated with follow-up 6-12 months, including 28 cases of cardiovascular events, no cardiovascular events of 60 cases of cardiovascular events in patients with AMI serum BNP levels significantly higher than those without cardiovascular events in patients with AMI BNP levels and QTc interval longer than without the occurrence of cardiovascular events in patients with AMI, the QTc interval (P 〈 0.05). Conclusion AMI patients with serum BNP levels are positively correlated with QTc. AMI disease condition monitoring by detection of plasma BNP levels can be used as a fast, simple and effective test indicators.
