摘要
目的 :观察血液透析患者补充左旋卡尼汀 (L - carnitine)是否能加强重组人红细胞生成素 (rh EPO)的作用。 方法 :将96例患者随机分为 3组进行治疗。 A组 :透析后静脉注入左旋卡尼汀 1.0 g;B组 :每周用 rh EPO 5 0～ 15 0 IU / kg,透析后皮下注射 1～ 2次 ;C组 :透析后静脉注入左旋卡尼汀 1.0 g,并且每周合用 rh EPO 5 0～ 15 0 IU / kg。治疗前及治疗后 2 ,4,6个月分别测定患者白蛋白、血红蛋白、红细胞体积分数。结果 :补充左旋卡尼汀的患者白蛋白明显高于单用 rh EPO者 (P<0 .0 1) ;2 ,4个月后 ,C组血红蛋白、红细胞体积分数明显高于 A,B组 (P<0 .0 1) ;C组 rh EPO的用量明显比 B组少 (P<0 .0 1)。结论 :左旋卡尼汀能改善血透患者的营养状况和血红细胞检测指标 ,其作用在 4个月时达高峰 ,并可明显减少患者使用 rh EPO的量。
Objective:To demonstrate whether L carnitine can further improve the effect of recombinant human erythropoietin (rhEPO) in treatment of anemia. Methods: The patients were randomly divided into 3 groups, group A : L carnitine 1.0 g intravenously infused after every dialysis session; group B: rhEPO 50 150 IU/kg subcutaneously injected once or twice a week; group C: L carnitine and rhEPO given together. The serum albumin, hemoglobin(Hb) and hematocrit(Hct) were monitored for 6 months. Results: The albumin value in L carnitine group was significantly higher than that in rhEPO group ( P <0.01). The Hb and Hct in group C were significantly hihger than those in group A and B ( P <0.01); the dosage of rhEPO in group C was significantly lower than that in group B ( P <0.01). Conclusion: L carnitine can improve the nutritional state and increase the Hb and Hct in patients with CRF, its effect reaches peak in the 4th month. The dosage of rhEPO in patients simultaneously given L carnitine can be largely reduced.
