慢性阻塞性肺疾病急性加重期T淋巴细胞的表达及其意义 被引量:5

摘要 目的:探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者外周血T淋巴细胞的表达变化及意义。方法:对本院ICU收治的40例AECOPD行机械通气的患者(AECOPD组)进行研究,测定其入院时和治疗后外周血Th1、Th2细胞及Treg细胞计数,并与健康体检人群(对照组)作比较。测定入院时降钙素原(PCT)值,计算入院24h内的APACHE-Ⅱ评分。结果:40例AECOPD组患者入院时Th1、Th2细胞计数明显升高,Treg细胞计数降低,与对照组相比有显著差异(P<0.01);出现PIC窗较早的患者,Th1、Th2细胞计数下降,Treg细胞计数上升,与入院时相比有显著差异(P<0.01);治疗效果不佳患者在病程第7天时测定的Th1、Th2细胞计数和Treg细胞计数与入院时相比无显著差异(P>0.05)。Th1、Th2细胞计数与PCT值及APACHE-Ⅱ评分呈正相关(P<0.01),Treg细胞计数与PCT值及APACHE-Ⅱ评分呈负相关(P<0.01)。结论:AECOPD患者感染早期存在细胞免疫异常,外周血Th1、Th2细胞和Treg细胞的表达将有助于了解患者免疫状态,判断病情严重程度。
