摘要
目的:观察静脉注射海洛因依赖者中HIV感染者心电图(ECG)的变化,并探讨海洛因依赖者中HIV感染与心电图改变的有效的因素,以便采取相关防治措施。方法:采用酶联免疫吸附法(ELISA)对海洛因依赖者血清进行抗-HIV抗体检测,阳性血清送广西疾病预防控制中心进行确认。对39例抗-HIV抗体阴性的海洛因依赖者(吸毒组)、60例抗-HIV抗体阳性的海洛因依赖者(HIV组)和30例健康体检人员(对照组)的心电图进行分析。结果:99例海洛因依赖者中共检出心电图异常者54例,其中吸毒组23例,占58.9%;HIV组31例,占51.7%。对照组中检出心电图异常者6例,占20.0%。吸毒组和HIV组的心电图改变率为76.9%和78.3%,分别与对照组(23.3%)比较差异均有统计学意义(均P<0.01)。HIV组的ST-T改变率高于吸毒组,差异无统计学意义(P>0.05)。吸毒组中20~29岁段的心电图改变率高于30~39岁段,差异有统计学意义(P<0.05)。吸毒组和HIV组中吸龄≤49个月段的心电图改变率均较高,两组中男性改变率(63.3%和54.5%)均高于女性(44.4%和43.8%)。18例海洛因依赖者合并多重心电图改变,20例海洛因依赖者存在多种药物滥用的情况。结论:海洛因依赖和HIV均对机体和心血管系统造成一定的损害,提示在抗HIV治疗中保护心血管系统的同时,应先予以进行戒毒后方开始为好。
Objective: To observe the changes of electrocardiogram (ECG) of HIV infections in the heroin addicts through vein inject heroin, and discuss the related factors between HIV infection in the heroin addicts and the changes of ECG, so as to take related prevention and control measures. Methods: Sixty people selected by way of cluster sampling survey were observed who were heroin addicts through vein inject heroin in the compulsory rehabilitation of nanning public security bureau, they were given serum resistance- HIV antibody test by the ELISA method (ELISA), and positive results were further confirmed by the Centers for Disease Control of Guangxi. A study on ECGs was carried on about 39 heroin addicts of serum resistance-HIV antibody test negative (drug group), 60 heroin addicts of serum resistance-HIV antibody test positive (HIV group) and 30 normal people (control group). Results. Fifty-four cases in 99 cases of heroin addicts were detected ECG disorder, 23 cases (58.9%) in drug group; 31 cases (51.7%) in HIV group, and 6 cases (20.0%) in control group. Changes of ECG in drug group and HIV group were 76.9% and 78. 3% respectively ( P 〈0.01 vs. control group: 23.3%). There was no statistical difference in the ST-T changes between HIV and drug groups. Changes of ECG in drug group were higher in age 20-29 than in age 30-39 years ( P 〈0.05). Changes of ECG in drug and HIV groups were higher in subjects with a history ≤ 49 months than subjects with a history 〉 49 months. Changes of ECG in drug and HIV groups were higher in men than in women (63. 3% vs. 44. 4% and 54.5% vs. 43.8% ; respectively). Eighteen cases of heroin addicts had multiple ECG changes, and 20 cases of heroin addicts abused multiple drugs. Conclusion: Both heroin addicts and HIV infections can cause damages of the body and the cardiovascular system on some degree, suggesting that we should protect cardiovascular system for the treatment of HIV infections at the same time, drug treatment should be carried out firstly.
