YAG激光虹膜周边切除术治疗不同程度晶状体半脱位继发性青光眼疗效分析 被引量:3

摘要 目的探讨YAG激光虹膜周边切除术对不同程度晶状体半脱位所继发性青光眼的有效性和持久性。方法选择排除前房积血、玻璃体积血、房角后退因素并确诊的具有虹膜膨隆、浅前房特点的晶状体半脱位继发性青光眼患者35例(35眼),通过超声生物显微镜(UBM)检查晶状体脱位范围将病人分3组,第1组晶体脱位<45°的15只眼,第2组脱位45°~90°的11只眼,第三组脱位90°~135°的9只眼分别行YAG激光虹膜周边切除术治疗。结果 3组病人虹膜周切成功后前房立即加深,治疗前后平均眼压比较及视力比较差异均有统计学意义(P<0.05);三组病人治疗后术后眼压差异有统计学意义(P<0.05);晶体脱离的范围与术后眼压相关(P<0.05)。结论 YAG激光虹膜周边切除术是治疗具有虹膜膨隆、浅前房特征的晶状体半脱位继发性青光眼的有效简便方法,同时晶体脱离的范围越小手术的效果越好。 Objective To explore the effectiveness and durability of YAG laser peripheral iridectomy in treatment of secondary glaucoma due to lens subluxation in various degrees. Methods Thirty five patients (35 eyes) with secondary glaucoma (35 eyes) due to lens subluxation having characteristics of shallow anterior chamber and iris bombe and excluded hyphema, vitreous hemorrhage were allocated in this study, and an- gle recession factors were examined by ultrasonic biomicroscopy (UBM) for dislocation of lens. They were divided into 3 groups. Patients in first group suffered with lens dislocation at 〈 45 o in 15 eyes, patients in second group suffered with lens dislocation at 45 - 90 in 11 eyes, and pa- tients in third group suffered with lens dislocation at 90-135 in 9 eyes. Patients in these 3 groups were treated with YAG laser peripheral iri- dectomy. Results Anterior chambers of patients in 3 groups were immediately deepened after treatment with successful peripheral iridectomy, their difference in intraocular pressure and visual comparison was statistically significant ( P 〈0.05 ) ; and the difference in postoperative intraoeu- lar pressure among patients in 3 groups after treatment was also statistically significant ( P 〈 0.05 ) ; and the detachment scope of lens was related to postoperative intraocular pressure ( P 〈 0.05 ). Conclusion The application of YAG laser peripheral iridectomy in treatment of secondary glaucoma due to lens subluxation with characteristics of iris bombe, shallow anterior chamber is an effective and simple method, meanwhile the less detachment scope of lens is, the better will be the effect of operation.
