新生儿缺血缺氧性脑病CT诊断与出生时Apgar评分的相关性研究 被引量:5

摘要 目的探讨新生儿缺血缺氧性脑病(HIE)CT影像学表现、分级与临床Apgar评分的内在联系,为患儿的后续治疗与康复提供参考。方法随机抽取我院2008年3月—2011年12月105例HIE患儿的临床及影像资料,并对其进行回顾性分析。结果临床上出生1min时Apgar评分≤3分者22例,占20.95%;3分<Apgar评分≤7分者57例,占54.29%;>7分者26例,占24.76%。HIE轻度26例;中度57例;重度22例。CT分度:轻度9例;中度43例;重度53例。结论 CT检查能清晰地显示病变的部位、范围及程度,其敏感性远高于临床。因此,应合理运用CT检查手段,诊断时更应密切结合临床,在充分把握和理解其病理生理演变过程的基础上,才能正确评估病变的性质、程度和预后。 Objective To investigate neonatal hypoxic - ischemic encephalopathy (HIE) CT imaging findings, classi- fication and clinical Apgar score of internal relations, provide a reference for follow - up treatment and rehabilitation of children. Methods The randomly selected from our hospital from March 2008 to 2011 in December of 105 cases of HIE clinical and ima- ging data, and were analyzed retrospectively. Results Clinically was born 1 rain Apgar score ≤3 in 22 cases, accounting for 20. 95% ; 3 〈 Apgar score ≤7 in 57 cases, accounting for 54. 29% ; 〉 7 in 26 cases, accounting for 24. 76%. 16 cases of light; 57 cases of moderate; 22 cases of severe. CT indexing: 9 cases of mild to moderate in 43 cases; 53 cases were severe. Conclusion CT can clearly show the location, scope and extent of lesions, its sensitivity is much higher than the clinical, therefore, should be a reasonable use of the means of CT scan, should be closely combined with clinical diagnosis, to fully grasp and understand the pathophysiclagy evolution on the basis of the process, to correctly assess the nature of disease extent and prog- nosis of trends.
