慢性阻塞性肺疾病患者肺功能急性加重期痰培养细菌学分析 被引量:11

Bacteriology of sputum culture for chronic obstructive pulmonary diseases patients in acute exacerbation stage

摘要 目的探讨慢性阻塞性肺疾病(COPD)患者不同肺功能的外周血浆脑利钠肽(BNP)变化以及COPD急性加重期痰细菌学状况。方法选择自2010年8月-2011年12月医院住院检查及治疗的COPD患者88例,分别测定患者的血浆BNP水平、肺功能以及肺动脉收缩压;根据肺功能以及肺动脉收缩压,分别探讨外周血浆BNP水平在不同严重程度COPD患者的变化以及肺动脉高压对其影响。结果不同严重程度COPD患者外周血浆BNP浓度相比,差异有统计学意义(P<0.01);COPD患者外周血浆中的浓度随着其严重程度递增;88例患者痰培养,共检出细菌53株,其中革兰阴性菌37株占69.81%;革兰阳性球菌16株占30.19%。结论外周血浆BNP水平与COPD患者肺功能病变严重程度有关,可作为评判COPD患者预后的潜在指标,COPD急性加重期患者,痰细菌学培养结果与患者肺功能损害关系密切。 OBJECTIVE To investigate the change of peripheral blood plasma brain natriuretic peptide(BNP) of different lung functions of chronic obstructive pulmonary disease(COPD)patients in COPD exacerbations and analyze the bacteriology of sputum culture in the COPD acute exacerbation stage.METHODS A total of 88 COPD patients who enrolled the hospital from Aug 2010 to Dec 2011 were selected as the study objects,the plasma BNP level,pulmonary function,and pulmonary artery systolic blood pressure were respectively determined;According to the lung function and pulmonary artery systolic pressure,the change of peripheral blood plasma BNP level in the patients with different severity of COPD and its impact on pulmonary arterial hypertension were observed respectively.RESULTS The difference in the peripheral blood plasma BNP concentration between the patients with different severity of COPD was statistically significant(P 0.01);The peripheral blood plasma BNP concentration increased with the increment of the severity of COPD.A total of 53 strains of bacteria were isolated from the sputum of 88 patients,including 37(69.81%) strains of gram-negative bacteria and 16(30.19%) strains of gram-positive cocci.CONCLUSION The peripheral blood plasma BNP level is associated with the severity of pulmonary function lesions of the COPD patients,which can be used as a potential index for evaluating the prognosis of the COPD patients.For the COPD patients in the acute exacerbation stage,the result of the sputum culture for bacteriology is closely related to the damage of pulmonary function of the patients.
