摘要
目的:探讨在2型糖尿病(T2DM)患者中25-羟维生素D3[25(OH)D3]浓度同下肢周围血管病变(PAD)的关系。方法:入选105例T2DM患者,根据下肢血管超声结果将患者分为PAD组(49例)和无下肢血管病变(Non-PAD)组(56例),并选取同期来本院体检的50例健康者为对照组。所有患者检测血脂、糖化血红蛋白C(HbA1c)、C反应蛋白(CRP)等血液生化指标,用ELISA法测定血清25(OH)D3的浓度,用彩色多普勒超声进行下肢血管检查。结果:T2DM组患者血清25(OH)D3浓度明显低于对照组,且在T2DM患者中PAD组25(OH)D3浓度明显低于Non-PAD组,差异均有统计学意义(P<0.05)。Pearson相关分析显示,PAD与年龄、吸烟、糖尿病病程、BMI、HbA1c、CRP、LDL-C呈正相关(r=0.56=0.16,均P<0.05),与25(OH)D3呈负相关(r=-0.49,P<0.01)。多元回归分析显示下肢血管病变与25(OH)D3浓度具有独立相关性(β=-0.37,P<0.01)。结论:血清25(OH)D3浓度与T2DM患者PAD发生有关。
Objective To investigate the relationship between 25- hydroxyvitamin D3 (25(OH)D3) and lower-extremity peripheral arterial disease (PAD) in type 2 diabetic (T2DM) patients. Methods A total 105 T2DM and 50 non-diabetic healthy controls were enrolled in this study. All the diabetic patients were randomized into 2 groups [PAD group (n = 49) and Non-PAD group (n = 56)] according to whether with PAD. Routine biochemical indexes were determined. Serum 25 (OH)D3 concentration was determined by ELISA. Lower extremity vascular was measured with Doppler ultrasound in all subjects. Results Serum 25 (OH)D3 concentration decreased significantly in patients with T2DM, and specially in PAD (P 〈 0.05). The result of Pearson's correlation analysis showed that PAD was positively correlated with age, smoking, duration of diabetes, BMI, HbAlc, CRP and LDL-C (r = 0.56 = 0.16, P 〈 0.05), but was negatively correlated with serum 25 (OH) D3 concentration (r = -0.49, P 〈 0.01). Low serum 25 (OH)D3 concentration was an independent risk factor for PAD based on multivariate regression analysis (13 = -0.37, P 〈 0.01 ). Conclusion Serum 25 (OH)D3 concentration was strongly and independently associated with PAD in patients with T2DM.
