摘要 目的探讨转化生长因子-βB1(TGF-β1)、基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶抑制剂-3(TIMP-3)在女性盆底功能障碍性疾病(PFD)患者阴道壁中的表达及三者与PFD发病的关系。方法研究组(PFD组)为28例患者阴道壁结缔组织标本,分别为绝经前PFD组17例,绝经后PFD组11例;对照组为22例无压力性尿失禁和盆腔器官脱垂者,分别为绝经前对照组12例,绝经后对照组10例,采用RT-PCR方法测定各组阴道壁组织中TGF-β1、MMP-3、TIMP-3mRNA的表达。结果PFD组患者MMP-3的表达较对照组显著升高,两组之间差异有统计学意义(P=0.000〈0.01),TIMP-3及TGF-β1的表达较对照组显著下降,两组之间差异有统计学意义(P值分别为绝经前0.035,0.024;绝经后0.016,0.017;均P〈0.05)。结论MMP-3高表达,TIMP-3作为MMP-3的抑制剂的低表达以及TGF-β1的低表达,使盆底胶原的降解增加,从而使弹性纤维减少,导致PFD的发生。 Objective To explore the expressions of transforming growth factor-β1TGF-β1, matrix metatloproteinase-3 (MMP-3) and tissue inhibitor of matrix metalloproteinase-3 ( TIMP-3 ) in vaginal wall tissue of women with pelvic floor dysfunction (PFD) and their relationship with the occurrence of PFD. Methods Twenty-eight samples of vaginal wall connective tissues of women with PFD were selected as study group (PFD group), including 17 cases in premenopausal PFD group and 11 in postmenopausal PFD group. Twenty-two women without stress urinery incontinence (SUI) or pelvic organ prolapse (POP) were selected as control group, including 12 eases in premenopausal control group and 10 in postmenopausal control group. Reverse transcription polymerase chain reaction (RT-PCR) was used to determine the expressions of TGF-β1 MMP-3 and TIMP-3 mRNA. Results The expression of MMP-3 in PFD group was significantly higher than that in control group, and the difference was significant (P 〈 0.01 ). But the expressions TIMP-3 and TGF-β1was significantly lower than those in control group, and there were statistically significant differences ( P value was 0.035 and 0.024 in premenopausal group and 0.016 and 0.017 in postmenopausal group, respectively, all P 〈 0.05 ). Conclusion The high expression of MMP-3, low expression of TIMP-3 as the inhibitor of MMP-3 and low expression of TGF-β1enhance the degradation of collagen in pelvic floor, which induces the decreasing of elastin fiber and the incidence of PFD.
