斑点追踪分析左心室前壁缺血心肌25个感兴趣区的局部心肌功能 被引量:3

Regional wall fuction of the anterior wall of the left ventricle analysed by twenty-five regions of interest using two-dimensional and speckle-tracking echocardiography

摘要 目的:运用斑点成像技术矩阵取样的方法在左心室前壁心肌额断面上,分析左室前壁心肌上25个感兴趣区(ROIs)的应变与应变率,观察左心室前壁缺血时左心室前壁额断面局部心肌功能的变化。方法:选择左心室前壁缺血(缺血组)患者及无左心室前壁缺血(对照组)的患者各20例,在左心室前壁心肌额断面超声采集25个ROIs的斑点追踪所获的收缩期应变(ε)、收缩期应变率(SRS)、舒张早期应变率(SRE)及舒张晚期应变率(SRA),比较两组患者应变及应变率的差异,并以对照组SRS、SRE及SRA的95%医学参考值范围作为阈值参考,计算缺血组异常ROIs的百分率(PAR)。结果:25个ROIs的斑点追踪测得缺血组SRS与SRE均小于对照组(P<0.05);以对照组应变率的测量值制定95%医学参考值范围为基准参数,观察到缺血组的左心室前壁心肌SRS与SRE低于对照组基准水平18.66%~25.35%。结论:应用左心室前壁心肌额断切面以矩阵采样的方式,采集25个ROIs的斑点追踪局部左室功能参数,通过心肌异常位点的检出率(PAR)能够检测出左心室前壁心肌缺血患者的局部心肌功能异常。 Objective:To measure myocardial strain and strain rate in a multiple sampling way at coronal plane of left ventricle (LV) in patients with ischemia in the anterior wall (AW) of LV using two-dimensional and speck- le-tracking echocardiography (STE). Method:Patients with or without myocardial ischemia in the AW of LV were selected (20 patients for each group). Twenty-five regions of interest (ROIs) on the AW were displayed and tracked by STE. Variability of the strain (e), strain rates in systole (SRS), strain rates in early-diastole (SRE), strain rates in late-diastole (SRA) were recorded. ε, SRS, SRE and SRA of the group without ischemia AW of LV were consideraded as 95% medical reference range for normal wall motion and percentage of abnormal ROIs (PAR) were calculated. Parameters under the reference were considered abnormal. Result..~, SRS and SRE by 25 ROIs of STE were siginificantly decreased in group of ischemia in the AW (P〈0.05). PAR of ε, SRS and SRE in group of i schemia of the AW were from 18.66% to 25.35 % as evaluated by reference in the control. Conclusion: Twenty-five sampling ROIs at coronal plane of LV in patients could be a valuable modality for detention of regional wall abmormal in paitents with normal global LV funcution.
