摘要
目的研究应用髁限制性假体治疗严重外翻畸形膝关节骨关节炎的临床效果。方法 2009年1月至2012年6月共收治12例严重外翻畸形膝关节骨关节炎患者,采用人工全膝关节置换术,使用髁限制性假体,术后早期进行膝关节功能锻炼。结果手术时间80～120min,平均(93±9.89)min,出血量200～600mL,平均(379±45.74)mL。手术切口一期愈合,未发生手术并发症。12例均获随访,时间3～30个月。HSS膝关节评分:优8例,良3例,差1例,膝关节稳定性良好。结论对于严重外翻畸形膝骨关节炎通过软组织平衡技术不能获得间隙平衡时,应用髁限制性假体能保证膝关节的稳定,延长假体的存留,获得良好的临床效果。
Objective To evaluate the clinical effect of condyle constrainted implant in the treatment of severe valgus knee osteoarthritis.Methods From January of 2009 to June of 2012,12 cases of knee osteoarthritis with severe valgus deformity were treated using condyle constrainted implant.Knee exercise began shortly after operation.Results The operation time was 80～120（93±9.89） min and the amount of blood loss was 200～600（379±45.74） mL.The patients had their incision healed by first intension and no complication occurred.All the12 patients were followed up for 3～30 months.With the function score of HSS,the results were excellent in 8,good in 3,and bad in 1.The knee joint stability was good.Conclusion Good curative effect,knee stability and long-term prosthesis retention can be got by using condyle constrainted implant for many severe valgus knee replacements with unobtainable gap balance through soft tissue balancing.
