摘要
目的:探讨血液灌流联合耳穴贴压治疗维持性血液透析(Manitenance Hemodialysis,MHD)并发症的疗效。方法:MHD患者90例随机分为治疗组和对照组1及对照组2,每组30例,治疗1个月,治疗组在血液透析(Hemodialysis,HD)基础上做血液灌流加耳穴贴压治疗,对照组1在透析基础上联合血液灌流治疗,对照组2仅行血透治疗。观察三组患者治疗前、后的实验室检查指标(肾功能、甲状旁腺激素、β2微球蛋白、电解质)变化与临床症状(皮肤瘙痒、骨痛、睡眠障碍、出汗异常)的变化。结果:治疗后,治疗组和对照组1各临床症状明显改善,且治疗组患者睡眠障碍和出汗异常改善显著多于对照组1,两者比较差异有统计学意义(P<0.05)。对照组2各临床症状未出现明显改善。治疗组和对照组1甲状旁腺激素、β2微球蛋白、钙、磷及肌酐、尿素氮水平与治疗前比较差异有统计学意义(P<0.05)。对照组2治疗前后肌酐、尿素氮水平比较差异有统计学意义(P<0.05),而甲状旁腺激素、β2微球蛋白、钙、磷水平比较差异无统计学义意义(P>0.05)。结论:血液灌流能有效清除甲状旁腺激素、β2微球蛋白等中的分子毒素,纠正钙磷代谢紊乱,联合耳穴贴压治疗能更明显缓解MHD病发症的临床症状,其疗效要优于单纯血液灌流和常规血液透析。
Objective：To study the effect of hemoperfusion joint auricular acupressure in treating the MHD complications.Methods：90 patients were randomly divided into three groups：treated group,control group 1 and control group 2,30 cases in each group.The period of treatment was one month.The treated group was given the treatment of hemoperfusion joint auricular acupressure;the control group 1 was given HD joint hemoperfusion;the control group 2 was given HD only.Changes of symptoms（skin itching,bone pain,sleep disorder,abnormal sweat） and lab testing datas（BUN,Scr,PTH,β2-MG,Ca,P） of 3 groups were observed and recorded before and after treatment.Results：After one month treatment,the symptoms of the treated group and control group 1 were improved,especially sleep disorder and abnormal sweat of the treated group were improved more significantly than those of control group 1,the differences were statistically significant（P0.05）.The symptoms of control group 2 were seldom improved.The levels of BUN,Scr,PTH,β2-MG,Ca and P of treated group and control group 1 were lower significantly than those before treatment（P0.05）.The levels of BUN and Scr of the control group 2 were lower significantly than those before treatment（P0.05）,but the levels of PTH,β 2-MG,Ca and P were not lower than before（P0.05）.Conclusion：Hemoperfusion could effectively clean the middle molecular substance and correct metabolic disorders of Ca and P,combined with auricular acupressure could significantly relieve the MHD complications,symptoms,and its effects were better than that of single hemoperfusion and HD.
