摘要
目的采用偱证医学Meta分析的方法以探讨临床路径对精神分裂症患者临床诊疗的影响。方法计算机检索中文数据库,全面收集有关国内临床路径对精神分裂症患者临床诊疗的影响临床对照研究,汇总数据采用RevMan5.1.6和STATA10.0进行分析,两组之间差异采用标准均数差(standardized mean difference,SMD)或优势比(oddsratio,OR)及其95%可信区间(95%confidence interval,95%CI)描述。结果严格根据纳入和排除标准,最终纳入5个临床对照研究,包括429例精神分裂症患者,其中观察组213例,对照组216例。Meta分析结果表明:临床路径组的平均住院时间、平均住院费用及患者满意度与对照组相比均有明显统计学差异(均P<0.05)。结论临床路径作为提高医疗护理质量、降低医疗护理费用的一种新的管理模式,在临床上值得推广应用。
Objective To investigate the effects of clinical pathway management applied on schizophrenics by conducted a systematic review and meta-analysis.Methods WanFang,CNKI,VIP and CBM databases were searched extensively to identify relevant clinical controlled trials.Data were analyzed by RevMan 5.1.6 and STATA 10.0 softwares.Results were described as Standardized mean difference （SMD）or odds ratio （OR） with 95% confidence intervals （CI）.Results Finally,a total of 3 clinical controlled trials involving 208 schizophrenia were included in this systematic review,including 105 patients in observation group and 103 patients in control group.Meta-analysis showed that there were statistically significant differences in patients＇ average length of hospital stay （P0.05）,average hospital costs （P0.05） and patient satisfaction （P0.05）.Conclusion The results of this study showed that the clinical pathway used on patients with schizophrenia,could significantly shorten the average length of stay,reduce hospital costs,improve medical care and nursing service quality and patients＇and their families＇cognitive ability to the disease and satisfaction to nursing action,relieve or improve patients＇psychiatric symptoms effectively at the same time,promoting patients＇rehabilitation.Clinical pathways,as a new model of management,should be applied widely to improve health care quality and reduce medical care costs in clinical practice.
