摘要
目的探讨脂肪干细胞(ADSCs)移植对心肌梗死(MI)后心电生理的效应及其机制。方法采用结扎冠状动脉左前降支的方法制作大鼠的心梗模型,将存活的大鼠随机分为:MI组(n=11),予注射磷酸缓冲液(PBS);ADSCs组(n=11),予注射ADSCs。另设假手术(sham)组(n=10),只挂线不结扎,余处理同MI组。2周后通过程序电刺激检测室性心律失常(VAs)的诱发及心室颤动阈值(VFT);通过masson染色观察梗死面积以及免疫荧光观察缝隙连接蛋白43(Cx43)的变化以及移植细胞的存活情况。结果与MI组比较,ADSCs组VFT明显增高(6.0±0.9 V vs 4.8±0.6 V,P<0.001),VAs的诱发率明显降低(36.4%vs 91.0%,P<0.001);MI组VFT较Sham组显著下降(4.8±0.6 V vs 6.5±0.8 V,P<0.001);sham组未见诱发出VAs。组织学检测显示ADSCs移植明显减少梗死面积(22.6%±2.3%vs 42.0%±2.6%,P<0.05)并部分纠正MI后Cx43表达(32.2±3.1 vs 19.2±0.8,P<0.01)下降及分布紊乱。结论 ADSCs移植可改善MI后电生理重构,可能与减少梗死面积并改善梗死周边区Cx43的表达和分布有关。
Objective To investigate the electrophysiological effect of intramyocardial injection of ADSCs in a rat MI model. Methods After coronary artery ligation, rats were randomized to receive intramyocardial injection of PBS alone (MI group, n = 11 ), ADSCs (ADSCs group, n = 11 );Another 10 rats were served as sham operation control (sham group). Two weeks after transplantation, ventricular arrhythmias (VAs) inducibility and ventricular fibrillation threshold (VFT) were assessed in vivo by programmed electrical stimulation. Histological analysis was used to observe infarct size, connexin43 distribution and cell rentention. Results Compared with MI group , ADSCs injection led to significantly raised VFT(6.5 ± 0.8 V vs 4.8 ± 0.6 V, P 〈 0. 001 ), reduced inducibility of VAs (36.4% vs 91.0% ,P 〈 0.001 ). The scar area in ADSCs group was significantly decreased compared with MI group(22.6% ± 2.3% vs 42.0% ± 2.6% ,P 〈 0. 05 ) and the average optical density of Cx43 in ADSCs group increased highly in comparison with MI group ( 32.2 ± 3.1 vs 19.2 ± 0.8 ,P 〈 0.01 ). Conclusion ADSCs transplantation,to some degree, can ameliorate myocardial electrical physi- ological status after MI. It's probably due to associated with its reducing in infarct area ,perfecting distribution and expres- sion of Cx43.
