摘要
目的探讨16层螺旋CT三维重建在下颌骨髁状突骨折诊断及分类中的应用价值。方法对43例髁状突骨折患者行螺旋CT扫描并进行三维重建后处理,将所得图像按骨折部位分为4类,按骨折片位移程度分为3型,并与常规X线平片及横断二维CT图像进行对比分析。结果螺旋CT三维重建能清楚地显示下颌骨髁状突骨折的部位及位移情况,显示率为100%,并能准确地进行分类及分型,在显示骨折及诊断准确性方面优于常规X线平片及横断二维CT图像。结论螺旋CT三维重建能准确地显示髁突骨折情况以及骨折的移位程度,在下颌骨髁状突骨折的诊断及分类方面具有很好的应用价值,应作为下颌骨髁状突骨折的常规检查方法。
Objective To evaluate the clinical value of three dimensional construction with 16-slice spiral CT in the diag- nosis and classification of mandibular condyle fractures. Methods A total of 43 patients with mandibular condyle fractures were scanned with spiral CT and three dimensional reconstructions were done. These images were classified to four types by the frac- tured position and three subtypes by the displacement of the fractured pieces. These three dimensional CT images were com- pared with X-ray images. Results Three dimensional construction with spiral CT could show the fracture of condyle and the displacement of the fracture clearly (the display rate was 100 ~ ) and classify accurately. Compared with radiography and 2D-CT, 3D-CT was superior in revealing fracture and diagnosing. Conclusion Three dimensional construction with spiral CT are essen- tial to demonstrate the fracture of condyle and the displacement of the fracture. Three-dimensional reconstruction has important application value for the diagnosis and classification of mandibular condyle fractures. Accordingly, three dimensional spiral CT can be taken as a routine ways for mandibular condyle.
