摘要 目的:探讨DHS常规内固定与微侵入手术治疗老年骨质疏松性股骨转子间骨折的临床疗效。方法:选择笔者所在医院2010年10月-2011年10月收治的72例老年骨质疏松性股骨转子间骨折患者,随机分为对照组和观察组,每组36 例。对照组采用DHS常规内固定法治疗,观察组采取DHS微侵入法治疗。比较术中出血量、手术时间、并发症及骨折愈合质量等指标及术后恢复情况。结果:与对照组比较,观察组的手术时间、切口长度、术中出血量、住院时间均明显降低(P<0.05);观察组的临床疗效也明显优于对照组(P<0.05)。结论:DHS微侵入内固定手术创伤小、减少软组织损伤,临床疗效确切、内固定安全可靠、并发症少,有助于提高患者术后的生活质量,适宜临床推广应用。 Objective: To explore the clinical effect of DHS conventional internal fixation and minimally invasive treatment of osteoporotic femoral intertrochanteric fractures in the elderly.Methods: 72 elderly patients with osteoporotic femoral intertrochanteric fractures from October 2010 to October 2011 in author's hospital were randomly divided into control and observation group, and each group of 36 cases.The control group was given conventional DHS open reduction and internal fixation while the observation group was received DHS minimally invasion.The operative blood lose(perioperative bleeding/blood loss during operation), operation time, complications, fracture healing quality and postoperative recovery were compared in the two groups.Results: The operative time, incision length and intraoperative blood loss(the amount of blood loss) and hospital stays in the observation group were significantly lower than the control group(P〈O.05); the clinical efficacy of the observation group was better than the control group (P〈0.05).Concluslon: The clinical efficacy of DHS minimally invasive fixation for treating with osteoporotic femoral intertroehanteric fractures in the elderly is simple, safe and reliable fixation with lower rates of complication, minimize surgical injury and risk, it can improve their quality of life and it should be vigorously promoted in the clinical application.
