选择性肾动脉栓塞治疗肾血管平滑肌脂肪瘤的临床效果 被引量：1

摘要 目的评价选择性肾动脉栓塞治疗肾血管平滑肌脂肪瘤（RAML）的安全性和有效性。方法21例RAML患者，其中肿瘤破裂9例，采用肾动脉造影加供血动脉栓塞术治疗。栓塞剂为无水乙醇、聚乙烯醇颗粒和（或）弹簧圈。结果20例患者成功完厩栓塞术。9例肿瘤破裂者术后即刻止血，其中3例失血性休克患者栓塞术后血压均逐渐恢复正常。1例因肿瘤多发且弥漫占据整个肾脏，血管造影未见舯瘤血管主干．治疗失败。全部患者随访1年以上，1例患者肾动脉造影复查时，肿瘤局部新生血管形成，行二次栓塞，其余患者均见肿瘤体积不同程度缩小，随访期间未出现肾周或腹膜后血肿。结论选择性肾动脉栓塞治疗RAML创伤小、恢复快，可有效防止肿瘤破裂出血，手术安全，最大程度保留肾单位，长期随访效果良好。
