不同糖耐量人群股动脉僵硬度、内中膜厚度及相关性 被引量:1

Study on femoral arterial stiffness and intima-media thickness among people with different glucose tolerance and their correlations

摘要 目的探讨糖代谢异常与股动脉僵硬度、内中膜厚度之间的关系。方法选取30例初诊2型糖尿病患者,糖耐量受损患者12例,正常糖耐量受试者20例,分别测定生化指标、股动脉内中膜厚度及股动脉僵硬度,比较分析各指标之间的相关性。结果与正常糖耐量组比较,糖耐量受损患者HOMA-IR(P<0.05)、股动脉内中膜厚度(P<0.05)及股动脉僵硬度(P<0.05或P<0.01)明显升高;2型糖尿病患者HOMA-IR、HbA1c、TG、股动脉内中膜厚度及股动脉僵硬度明显升高。与糖耐量受损组比较,2型糖尿病患者HbA1c、TG及股动脉僵硬度明显升高(P<0.05);两组股动脉内中膜厚度未见明显差异。不同糖耐量人群股动脉僵硬度与年龄、吸烟、SBP、HbA1c之间呈明显正相关(P<0.05);股动脉内中膜厚度与年龄、SBP、HbA1c、TC之间呈正相关(P<0.05);多元回归分析显示BMI、HbA1c为股动脉僵硬度的独立危险因素(P<0.05),SBP、HbA1c为股动脉内中膜厚度的独立危险因素(P<0.05)。结论股动脉僵硬度及股动脉内中膜厚度在糖尿病前期已有不同程度增加,可作为筛查糖尿病外周血管早期病变的可靠指标,而前者尤为灵敏。糖尿病外周血管病变需予以控制血糖为基础的全方位综合干预。 Objective Objective To discuss the correlation between glycometabolism disorder and femoral arterial stiffness and intima - media thickness. Methods Thirty patients who were preliminarily diagnosed as type 2 diabetes, 12 ones with glycometabolism disorder, and 20 people with normal glucose tolerance were chosen to receive the detection of their biochemical indicators, femoral arterial intima - media thickness,and femoral arterial stiffness. The correlations among different indicators were compared and analyzed. Results Compared with the indicators of people with normal glucose tolerance, the HOMA - IR ( P 〈 0.05 ), femoral arterial intima - media thickness ( P 〈 0.05), and femoral arterial stiffness ( P 〈 0. 01 ) of patients with glycometabolism disorder increased significantly; the HOMA - IR( P 〈 0. 01 ), HbAlc ( P 〈 0.05 ), TG( P 〈 0.01 ), femoral arterial intima - media thickness ( P 〈 0.05 ), and femoral arterial stiffness(P 〈 0.01 ) of patients with type 2 diabetes increased significantly. Compared with the indicators of patient with glycometabolism disorder,the HbAlc( P 〈 0.05 ), TG ( P 〈 0.05 ), and femoral arterial stiffness ( P 〈 0.05 ) of patients with type 2 diabetes increased significantly,but there was no significant difference in the femoral arterial intima - media thickness between the two groups. Among people with different glucose tolerance,there was a significant positive correlation between the femoral arterial stiffness and age, smoking, systolic blood pressure (SBP), and HbAlc (P 〈 0.05 ); there was a positive correlation between femoral arterial intima - media thickness and age, SBP, HbAlc ,and TC(P 〈 0.05 ). The multiple regression analysis showed that BMI and HbA1 c were independent risk factors of femoral arterial stiffness (P 〈 0. 05 ), and SBP and HbA1 c were independent risk factor of femoral arterial intima- media thickness (P〈 0. 05 ). Conclusion Femoral arterial stiffness and femoral arterial intima - media thickness have increased in different degrees in the early stage of diabetes, which, especially the former one, can be used as the reliable indicators for the screening of peripheral arterial vasculopathy. Based on the control of blood sugar, comprehensive intervention should be carried out on the peripheral arterial vasculopathy in patients with diabetes mellitus.
