摘要
目的:研究CEA、NSE、CA125及CA199在孤立性肺结节中的表达情况,探讨其在孤立性肺结节中的诊断价值。方法:选择2010年1月~2011年12月间经胸部X线、CT诊断的孤立性肺结节患者70例,其中术后经病理证实为肺癌的患者38例作为肺癌组,良性病变患者32例作为良性病变组,并选择同期体检健康人30例作为对照组,检测所有入组人员血清中癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、CA125及CA199水平。结果:肺癌组CEA、NSE水平明显高于良性病变组和对照组(P<0.05)。肺癌组CA125及CA199水平与良性病变组比较差异无统计学意义(P>0.05)。良性病变组CEA、NSE、CA125及CA199水平与对照组比较差异无统计学意义(P>0.05)。以CEA≥5.0ng/mL作为阳性参考值诊断肺癌其敏感度为57.9%,特异度为91.9%;以NSE≥15.2ng/mL作为阳性参考值诊断肺癌敏感度为44.7%,特异度为90.3%。两项联合敏感度为76.3%,特异度为93.5%。结论:CEA和NSE在肺癌患者中水平异常升高,对于孤立性肺结节的辅助诊断有一定的价值。
Objective. To study the expression of CEA, NSE, CA125 and CA199 in solitary pul- monary nodules, and to explore its value in diagnosing solitary pulmonary nodules. Methods:A total of 70 cases of solitary pulmonary nodules confirmed by chest X-ray, CT diagnosis were selected, including 38 cases of lung cancer patients as the lung cancer group, benign lesions of 32 patients as benign lesions group and 30 cases of healthy people the same period as the control group . Carcinoembryonic antigen (CEA), neuron-specific enolase enzyme (NSE), CA125 and CA199 level were detected. Results. The lung cancer group, CEA, NSE levels significantly higher than the benign group and the control group (P〈0.05). There was no significant difference in CA125 and CA199 level between lung cancer group and benign group (P〉0.05). CEA, NSE, CA125 and CA199 levels in benign lesions and t control group showed no signifi-cant difference (P〉0.05). As a positive reference value, the sensitivity of CEA ≥5.0 ng/mL was 57.9% in diagnosis of lung cancer, specificity was 91.9%while sensitivity of NSE ≥ 15.2 ng/mL was 44.7~, specificity was 90.3%. The sensitivity of combination was 76.3%, specificity was 93.5%. Conclusions. CEA and NSE levels are abnormally elevated in patients with lung cancer and has a certain value in diag- nosis of solitary pulmonary nodule.
