摘要
目的探讨骨转换生化标志物在早期诊断实体肿瘤骨转移的应用价值。方法定量检测58例原发实体肿瘤患者和32例健康人血清中骨转换生化标志物总Ⅰ型前胶原氨基端延长肽(TPⅠNP)、Ⅰ型胶原羧基端肽β特殊序列(β-CTx)、完整甲状旁腺素(iPTH)及碱性磷酸酶(ALP)等相关生化指标,用SPECT核素骨扫描确定有无骨转移。非参数Spearman相关分析骨转换标志物之间相关性。结果骨转移组TPⅠNP、β-CTx、iPTH、ALP水平均显著高于正常对照组(P<0.01);骨转移组TPⅠNP、β-CTx、iPTH水平均高于无骨转移组(P<0.05)。TPⅠNP和β-CTx之间有很好的相关性(r=0.453,P<0.01)。结论联合检测骨TPⅠNP和β-CTx在实体肿瘤骨转移的早期诊断中有重要应用价值。
Objective To investigate the value of biochemical markers of bone turnover in early diagnosis of bone metastasis of solid tumors. Methods Serum levels of total procollagen type I amino-terminal propeptide ( TP I NP), β-isomerized carboxyterminal propeptide (β-CTx), intact parathyroid hormone(iPTH) and alkaline phosphatase(ALP) were detected in 58 patients with primary solid tumors, who were divided into two groups of A(36 cases with bone metastasis) and B (22 cases without bone metastasis), and 32 healthy subjects (group, C). Single photon emission computed tomography(SPECT) radionuclide bone scan was performed to confirm bone metastasis. The correlations among the biochemical markers of bone turnover were analyzed with Spearman. Results Serum levels of TP I NP,β-CTx, iPTH and ALP were higher in group A than those in group C(P〈0. 01). Serum levels of TPI NP,β-CTx and iPTH were higher in group A than those in group B(P〈0. 05). There was a significant correlation between TP I NP and β-CTx( r= 0. 453, P〈0. 01). Conclusion Combined detecting serum TPINP and β-CTx is valuable in the early diagnosis of bone metastasis tumor of solid tumors.
