摘要
目的观察自发性脑出血患者小肠黏膜变化及其与胃泌素水平动态变化的关系。方法 25例自发性脑出血患者发病后第7天胶囊内镜观察小肠黏膜改变并使用酶联免疫分析法检测发病后第1,3,7,14,21天血清胃泌素浓度,并与11例健康志愿者对照。结果急性自发性脑出血患者发病后血清胃泌素水平升高,峰值时间为第7天。其中,非丘脑出血者及无小肠黏膜病变者血清胃泌素水平在第21天降至正常水平,而丘脑出血者或伴明显小肠黏膜病变者,在第21天时血清胃泌素仍高于正常对照组(P<0.05)。25例患者中13例小肠黏膜有不同程度的充血水肿、糜烂或浅溃疡形成,1例合并有活动性出血,对照组1例见小肠黏膜节段性充血(P<0.05)。结论自发性脑出血与小肠黏膜改变及胃泌素动态变化有着密切相关性。
Objective To explore the relationship between the changes of mucous membrane of small intestine and serum gastrin in patients with spontaneous cerebral hemorrhage.Methods The changes of the mucous membrane of small intestine were observed by capsule endoscopy in 25 patients with spontaneous cerebral hemorrhage at day 7,and serum levels of gastrin were measured by enzyme-linked immunoassay at day 1,3,7,14 and 21 after cerebral hemorrhage,respectively.Eleven healthy volunteers were chosen as controls.Results The level of serum gastrin increased after cerebral hemorrhage.The serum gastrin peaked at day 7 in patients with spontaneous cerebral hemorrhage,and decreased to the normal level at day 21.For these patients with no thalamic hemorrhage or no pathological changes of small intestine,the level of serum gastrin reduced to the normal level at day 21 after cerebral hemorrhage,while for the patients with thalamic hemorrhage or small intestine symptoms,the level of gastrin was higher than that in controls(P0.05).In 25 cases,15 were presented with hyperemia and punctiform erosion or superficial ulceration at different degree and one was active bleeding(P0.05),while there was one case of segment hyperemia in 11 controls.Conclusion Spontaneous cerebral hemorrhage may be related to the changes of mucous membrane of small intestine and dynamic change of serum gastrin.
