摘要
目的:对成人骨性Ⅱ类错患者治疗前头颅侧位片进行测量研究,分析切牙代偿情况及其在不同垂直骨面型中的差异,为临床诊治提供一定指导意义。方法:收集骨性Ⅱ类成人患者共200例,确定13项代表切牙倾斜度和凸度的指标进行X线头影测量分析,将结果与正常牙合均值比较,然后依照SN/MP角和FH/MP角将患者分为不同的垂直骨面型共3组,单因素方差分析各指标在组间的差异。结果:代表上切牙唇倾度的大部分指标测量均值(U1/SN、U1/NA、U1/FH)与正常牙合均值基本一致,L1/GoMe、L1/NB(°,mm)明显大于正常牙合均值;3组间比较,除U1/pp、U1/NA(°)及U1/NA(mm)外,其余的10项均具有统计学差异。结论:成人骨性Ⅱ类错患者上切牙相对颅部参考平面无明显的倾斜代偿但相对上颌基骨较为直立,下前牙唇向倾斜代偿特别明显;高角组上切牙直立,有一定舌向代偿,内收上前牙时要特别注意转矩控制;低角组下切牙唇倾度大,进一步正畸唇倾代偿的潜力小;随着下颌角平面的增高,上下切牙相对颏部的位置更加靠前,通过牙齿的代偿很难协调后缩的颏部。治疗中应结合Ⅱ类骨骼矢状不调程度和牙齿代偿在不同垂直骨型中的特点,明确牙齿进一步代偿或去代偿的潜力。
Objective X-ray cephalometric measuring for the adult patients with skeletal class Ⅱ before treatment, analysis incisive compensatory and the differences among three vertical skeletal type,provide the clinical diagnosis and treatment a certain guiding. Methods Total 200 skeletal class Ⅱ adult patients collected and the data of thirteen items which representative incisive gradient and convex degree were compiled to analyze the difference among three vertical skeletal type. Results Except the indexs of U1/pp,U1/NA （~） and U1/NA （mm）,others had stasitic significance in three groups. Conclusion Lower incisors had obvious compensatory characteristics in skeletal class Ⅱ adult patients, lingually inclined more seriously in group of low mandible angle compared with the group of high mandible angle, possibility for further compensatory were smaller. Upper incisors in lower mandible angle patients had no distinct compensatory compared with the group of high mandible angle patients.
