摘要
目的评价丙泊酚配伍地佐辛用于膀胱镜检查的效果。方法选择60例ASAⅠ或Ⅱ级的患者,随机分为3组,每组30例:F组静注芬太尼1μg/kg;D组静注地佐辛0.1mg/kg;N组静注生理盐水2ml。5min后3组静注丙泊酚观察患者睫毛反射消失后开始检查。观察3组患者给药前(T0)、入睡时(T1)、膀胱镜操作时(T2)、术毕时(T3)的MAP、HR、RR、SpO2。记录丙泊酚总用量、手术时间、苏醒时间、苏醒时的尿道痛视觉模拟评分(VAS),观察不良反应。结果 T1时点各组MAP、HR匀均显著低于T0时点(P<0.05),而组间差异无统计学意义;F组的RR在T1时点明显低于D组和N组(P<0.05)。F、D两组丙泊酚总用量、苏醒时间与N组相比显著减少(P<0.05),与N组相比苏醒时的尿道痛VAS评分D、F两组明显降低(P<0.05);恶心、呕吐的发生率F组明显高于D组及N组(P<0.05)。头昏及术中体动发生率F组及D组明显低于N组。结论丙泊酚配伍地佐辛用于膀胱镜检查,麻醉效果满意,呼吸抑制少,术后镇痛效果好,值得临床推广。
Objective To evaluate the effects of propofol combined with dezzocine for cystoscopy. Methods Sixty ASA I or lI patients who undergoing cystoseopy were randomly divided into three groups with 20 patients each: Group F:intravenous inject fentanyl μg/kg; Group D : intravenous inject dezocine 0. I mg/kg ; Group N : intravenous inject physiological saline 2 nil. 5 nfinutes later intravenous injected propofol until lost eyelash reflex and start the operations. MAP, HR, RR and SpO2 were observed at four time point : before anesthesia( To ) , after falling asleep( TI ) , intraoperation( T2 ) and postoperation (T3 ). The total a- mounts of propofol , time of wake-up, operation time, and VAS score of pain after consiousness were recorded. The adverse reac- tions during anesthesia and complications after anesthesia were observed. Results MAP and HR at TI time point were significant- ly lower than that at To time point in three groups (P 〈0. 05 ), but the comparative differences between groups was no statistically significant. RR of group F at T~ time point was significantly lower than that of groupD and N ( P 〈 0.05 ). The total amounts of propofol and time of wake-up of group F and D were significantly reduced than group N ( P 〈 0. 05 ). VAS score of pain after con- siousness of group F and D was significantly lower than that of group N( P 〈0.05 ) , The incidences of postoperative nauseaand vomting were significantly higher in group F than in group D and N ( P 〈 0. 05 ), The incidences of intraoperative body movement and postoperative dizziness were significantly lower in group F and D than in group N (P 〈 0. 05). Conclusion Dezocine combined with propofol for cystoscopy has satisfactory anesthesia effect and respiratory inhibition less , postoperative analgesia, it worth clinical promotion.
