新诊断2型糖尿病患者血清高迁移率族蛋白1和Toll样受体4水平变化及其与胰岛素抵抗的关系研究 被引量:5

Relationship between Serum HMGB-1 and TLR4 Level and Insulin Resistance in Newly Diagnosed Type 2 Diabetic Patients

摘要 目的探讨新诊断2型糖尿病(T2DM)患者血清高迁移率族蛋白1(HMGB1)和Toll样受体4(TLR4)表达水平及其与胰岛素抵抗的关系。方法选取2011年6—11月在我院内分泌科就诊的新诊断T2DM患者60例(T2DM组)及同期在我院体检的健康者30例(NC组)。观察两组受检者的空腹血清HMGB1和TLR4水平,并计算其胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、胰岛素敏感性指数(ISI),并将HMGB1、TLR4与其他观察指标(如血糖、血脂、肝功能、胰岛素)进行单因素相关分析,然后进行多元逐步回归分析。结果(1)T2DM组患者血清HMGB1水平为(89.8±13.4)μg/L,TLR4水平为(35.8±3.1)μg/L,NC组分别为(58.8±10.4)μg/L和(21.6±3.6)μg/L,两组HMGB1、TLR4水平比较差异均有统计学意义(t=11.059,19.373;P<0.01)。(2)单因素相关分析显示,血清HMGB1及TLR4均与体质指数(BMI)、三酰甘油(TG)、空腹血糖(FPG)、胰岛素抵抗指数(HOMA-IR)呈正相关,而与HOMA-β、ISI呈负相关,且HMGB1与TLR4亦呈正相关(P均<0.05)。(3)多元逐步回归分析发现,TLR4为影响HMGB1的最显著因素(β=0.281,P=0.000);HMGB1、HOMA-β、ISI为影响TLR4最显著的因素(β值分别为-4.146、-4.325和-5.061;P<0.05)。结论 T2DM患者血清HMGB1、TLR4均显著升高;且血清HMGB1与TLR4呈正相关;血清HMGB1与TLR4水平升高可能参与了胰岛素抵抗及T2DM的发生、发展。 Objective To explore the levels of serum HMGB1 and TLR4 as well as their relationship with insulin resistance(IR) in newly diagnosed T2DM patients. Methods 60 newly diagnosed T2DM patients admitted to our hospital from June to November in 2011 were selected as T2DM group and 30 healthy people were selected as NC group.The fasting HMGB1 and TLR4 levels of the two groups were observed and HOMA-IR,HOMA-β and ISI were calculated.Single factor analysis was performed on HMGB1,TLR4 and other observation indexes(blood glucose,blood lipids,liver function and insulin),and then multi-factor regression analysis was also performed. Results(1) The serum HMGB1 and TLR4 levels in T2DM group were(89.8±13.4) μg/L and(35.8±3.1) μg/L respectively,and in control group were(58.8±10.4) μg/L and(21.6±3.6) μg/L.The difference was statistically significant(t=11.059,19.373;P0.01).(2) Single factor analysis showed that serum HMGB1 and TLR4 were positively correlated with BMI,TG,FPG and HOMA-IR,but were negatively correlated with HOMA-β and ISI,and HMGB1 was also positively correlated with TLR4(all P0.05).(3) Multi-factor regression analysis showed that TLR4 was the most affective factor for HMGB1(β=0.281,P=0.000),and HMGB1,HOMA-β and ISI were the most affective factors for TLR4(β=-4.146,-4.325,-5.061;P0.05). Conclusion Serum HMGB-1 and TLR4 levels,which are positively correlated,have a significantly increase in T2DM patients.The increase of HMGB-1 and TLR4 levels may be involved in the occurrence and development of IR and T2DM.
