血液透析治疗急性肾功能衰竭18例临床观察 被引量:8

摘要 目的研究连续静脉-静脉血液透析滤过(CVVHDF)治疗急性肾功能衰竭(ARF)的临床效果,为临床该类疾病的治疗提供依据。方法选择2007年12月~2011年12月本院收治的36例体外循环下冠状动脉旁路移植术后导致的急性肾功能衰竭患者,将患者随机分为两组,观察组采用静脉-静脉血液透析滤过治疗,对照组患者采用非透析疗法,分析比较治疗前后两组患者的生理生化指标,包括心率、血压、血肌酐(Scr)、尿素氮(BUN)等,并比较两组患者的住院天数、并发症情况,以及转归情况。结果经过治疗,两组患者的生理生化指标与治疗前比较均有明显改善,差异有统计学意义(P<0.05);治疗后,两组患者的心率、血压、Scr以及BUN比较,差异有统计学意义(P<0.05);观察组患者平均住院时间[(16.8±5.6)d]少于对照组[(26.8±6.1)d],并且观察组患者术后并发症的发生率为16.7%,低于对照组,差异有统计学意义(P<0.05);观察组患者治愈率高于对照组,死亡率低于对照组,差异有统计学意义(P<0.05)。结论连续性静脉-静脉血液透析滤过治疗急性肾功能衰竭临床疗效显著,可明显改善患者生理指标,减少并发症发生率,缩短住院时间,治愈率及死亡率均好于非透析治疗,值得临床推广应用。 Objective To study the clinical effect of continuous veno-venous hemodiafiltration (CVVHDF) on acute renal failure(ARF), and provide the basis for clinic. Methods Thirty-six patients of ARF following coronary artery bypass grafting from December 2007 to December 2011 were divided into observation group and control group. Observation group was treated with CVVHDF and control group was treated with non-hemodiafiltration program, the physiological and biochemical indexes of the two groups including heart rate, blood pressure, Scr ,BUN, hospital stays and the outcome of treatment were compared. Results After treatment, all the indexes were changed compared to that before treatment, the difference were statistically significant(P 0.05), compared with control group, observation group changed significantly, the differences were statistically significant(P 0.05); hospital stays of observation were 16.8 d, which was lower than that of control group, and the rate of side effect was also lower than that of control group, which had statistically significant between the two groups(P 0.05); the cure rate of observation group was higher than control group and the mortality rate was lower, the differences were statistically significant(P 0.05). Conclusion CVVHDF show satisfactory clinical efficacy in the treatment of ARF, it can change indexes significantly, and reduce the incidence of complications and hospital stays, it is better than non-hemodiafiltration, so it is worthy of clinical use.
