摘要
目的讨论颈部猫抓病性淋巴结炎临床病理特点,以提高临床病理对猫抓病的认识。方法对23例颈部猫抓病性淋巴结炎的临床表现、病理特点和治疗进行回顾分析。结果 23例猫抓病性淋巴结炎均有狗猫等宠物接触史,其主要病理特征是淋巴结微脓肿性肉芽肿性炎,Wartin-starry银染色(+)。结论猫抓病性淋巴结炎临床表现多种多样,极易误诊,目前诊断主要靠详细询问病史及病理组织学诊断;治疗应早期应用抗生素可缩短病程,必要时行淋巴结切除术。
Objective To study the pathologic characteristics of cervical lymphadenitis in cat-scratch disease （CSD） in order to increase the awareness in clinical pathology. Methods The pathologic presentations, and therapeutic effects of cervical lymphadenitis in 23 cases of CSD were analyzed retrospectively.Results All patients with CSD had a history of contact with cats or dogs. The major pathologic features of CSD were granulomas associated with small abscess formation and the positive expressions of Warthin-Starry silver stains. Conclusion The clinical manifestations of lymphadenitis in cat-scratch disease were complex and varied. It is easy to result in a fault diagnosis in clinical work. At present,the final diagnosis of lymphadenitis in cat-scratch disease should be made by pathology.Early antibiotic usage can get a satisfactory the rapeutic effect,lymphnode should been cutted of necessary.
