The Third Force-Medical rescue effort in protecting public safety in metropolitan communities

摘要 本文以科学发展观为统领,回顾了当代国内外常态下和突发灾害事件救援的历史背景,介绍了发达国家城市急救体制与运行的模式,指出了我国急救的软肋,提出了在我国城市社区保障公众生命安全的急救医疗服务体系“第三支力量”的医学救援论点和实施。 Guided by the concept of scientific development, historical background of domestic and international medical rescue service for both normal and emergency state was retrospectively reviewed, and the model of metropolitan emergency system and its operation in those developed countries was introduced in this paper. Furthermore, with understanding of the weakness in our medical rescue mission, the concept of "Third Force", a medical rescue system which serves as the safeguard to our public safety, is being introduced, as well as its implementation. We all appreciate peacefulness which police force makes great effort in public safety, and a reduced loss which firemen strived in a disaster. But on-site medical rescue is still a weak part in our present medical service. Back to the 80s', an integrity of which police, fireman, and medical rescue personnel teamed up in handing emergency incidents in normal state emerged in the Western countries, Japan, and Hong Kong (special administrative region) with feature of being efficient in taking emergency calls and dispatching. For instance, emergency medical service (EMS), an integral part in the 911 system in the United States, carries out more work load than either police or firemen in normal state. It has been entitled as one of the three forces that keep the society peaceful. Once a major disaster occurred, the three forces carried out a significant integral effort with sufficient resources from different aspects of the society. The integral effort has been proved in the 911 incident back to 2001 in the U.S., of which the paramedic and the emergency technician had received full praise from the authorities and the whole society. While the Western countries have been working towards to perfecting their EMS system, we were stillstruggling on walking out of hospital' s gate, resulting in no progress made in developing modem EMS. Therefore, considering our actual condition and the proven experience in the Western countries and Hong Kong special administrative region, the opportunity of reform is in our hand - create the "Third Force". The current emergency systems have to be switched to emergency medical service (EMS) to position themselves for both normal and emergency states. Popularize first aid awareness, including training "first witnesses", promoting community safety, as well as self-save and mutual-save. Such intensive reform can be called "emergency medical revolution". Indeed, not only does it require a joint effort from both medical and public safety perspectives, but also the full support from different administrative levels, including their guidelines, supervision on the implementation, as well as all the medical personnel' s participation.
