多平面经食管超声心动图在经胸小切口室间隔缺损封堵术中的应用 被引量:10

Application of multiplane transesophageal echocardiography in device closure of ventricular septal defects with a minimally invasive technique

摘要 目的探讨多平面经食管超声心动图(MTEE)在监测经胸小切口室间隔缺损(VSD)封堵术中的应用价值。方法对术前经TTE和MTEE筛选的22例VSD患者行经胸小切口封堵术,在MTEE下完成治疗全程,包括术前封堵器选择、术中鞘管输送、封堵器释放及术后即刻评价封堵效果。结果 22例患者全部封堵成功,应用对称型膜部伞19例,细腰宽边伞3例。患者均于术后3~7天出院,随访1~6个月,无封堵器脱落、移位、溶血等并发症发生。结论经胸小切口VSD封堵术具有创伤小、恢复快、不用体外循环,避免X射线损伤的优势;MTEE对筛选患者、选择封堵器、指导封堵器释放及术后评价有重要的作用。 Objective To assess the value of multiplane transesophageal echocardiography (MTEE) in device closure of ventricular septal defects with aminimally invasive perventricular technique. Methods Twenty-two VSD patients were se- lected by TTE and MTEE before operation. Aminimally invasive surgical closure was performed on all the patients under the guidance of MTEE, including occluder selection, catheter sending, occluder release and evaluation of the effect imme- diately after operation. Results The procedures were successful in all 22 cases. Equilateral occluders were used in 19 and eccentric occluders in 3 cases. All the patients were out o{ hospital 3~7 days afer operation. During the 1~6 months fo[ low-up period, no complication such as Amplatzer defluvium, Amplatzer displacement, hemolysis occurred. Conclusion @ Minimally invasive transthoracic device closure of VSD has beautiful incision without CPB, with less trauma, quick re- covery and not affected by X ray radiation; (2) MTEE, as a safe and simple method, plays an important role in selecting patients and determining the size of the oecluder correctly before occlusion operation, guiding the placement of the occluder in operation and evaluating the effect after operation.
