结核感染T细胞斑点试验在结核诊断中的应用 被引量:10

摘要 目的比较结核感染T细胞斑点试验(T SPOT.TB)和结核菌素皮试试验(TST)在结核诊断中的敏感性和特异性,探讨两者在结核感染中的诊断价值。方法对疑诊或待排结核的68例患者进行前瞻性研究,分别采用T SPOT.TB和TST试验,比较二者对结核诊断的敏感性、特异性、阳性预测值(PPV)、阴性预测值(NPV)、阳性似然比(LR+)和阴性似然比(LR-)。结果 68例疑患结核患者确诊结核48例。T SPOT.TB实验在敏感性、特异性、PPV、NPV等方面显著优于TST(P<0.05);T SPOT.TB的LR+值明显高于TST,LR-值明显低于TST。结论 T SPOT.TB检测是诊断结核的快速敏感方法,显著优于结核菌素试验(TST),更适于作为结核筛检试验,在结核疾病诊断中有着重要的价值。 Objective To compare the sensitivity and speficity in diagnosis of tuberculosis by interferon-gamma release assy( T-Spot. TB) to the tuberculosis skin test( TST), in order to discuss their diagnositic values. Methods 68 patients suspec- ted to be infected with tuberculosis were taken into study. They were individually tested by T-Spot. TB and Tuberculosis Skin Test. When their diagnosis were certain, the sensitivity, speficity, positive prospective value, negative prospective value, positive likelihood ratio and negative prospective ratio of the two different diagnosis method were calculated. Results Ultimatidely 48 pa- tients were confirmed to have tuberculosis infected. The T-SPOt. TB apperanced abvious advantages in sensitivity, speficity, positive prospective value, negative prospective value, positive likelihood ratio compared to TST. Conclusion The T-Spot. TB is a quicker and more sensitive method in the diagnosis of tuberculosis compared to TST. It is valueable and applicable as a screening test.
