正畸拔除下颌第二双尖牙对下颌第三磨牙角度的影响 被引量:5

摘要 目的:比较下颌第二双尖牙拔除和非拔牙的正畸病例中,发育中的下颌第三磨牙角度的变化,以确定下颌第二双尖牙拔除是否有利于下颌第三磨牙角度的改善,从而有利于下颌第三磨牙的萌出。方法:从近3年已完成正畸治疗的患者中(采用直丝弓矫治技术)选取93名,分为两组,第一组为45名下颌第二双尖牙拔除者,第二组为48名非拔牙矫治病例,治疗前、后均已常规拍摄曲面断层片,然后测量并比较治疗前、后下颌第三磨牙相对于参考平面的角度改变。结果:拔牙组与非拔牙组患者矫治前后下颌第三磨牙角度均有不同程度的改善。拔牙组右侧下颌第三磨牙角度变化(5.9±2.5)度,拔牙组左侧下颌第三磨牙角度变化(8.0±3.4)度,非拔牙组右侧下颌第三磨牙角度变化(1.8±2.4)度,非拔牙组左侧下颌第三磨牙角度变化(1.5±2.1)度。与非拔牙组相比,拔牙组患者下颌第三磨牙角度变化量更大,差异有统计学意义(P<0.05)。结论:正畸拔除下颌第二双尖牙对下颌第三磨牙的角度有积极的影响。 Objective:To compare the angular changes mandibular second premolar extraction and non-extraction in the developing mandibular third molars in the cases and to determine whether the mandibular second premolar extraction causes favorable rotational changes in the angulations of the developing man- dibular third molars and benefits its later eruption. Methods: Pretreatment and posttreatment panoramic radiographs were taken from 45 subjects who had been treated by the extraction of the mandibular second premolars and 48 subjects who had used to measure and compare the c been treated without extraction. The horizontal reference plane was hanges in the angulations of the developing mandibular third molars. Results: The mean uprighting of the mandibular third molars seen in the extraction group was (5.9 -+ 2.5 ) degrees on the right side and (8.0 + 3.4) degrees on the left side following treatment. For the non- extraction group the mean uprighting was ( 1.8 + 2.4) degrees on the right side and ( 1.5 + 2.1 ) degrees on the left side. There was a statistically significant difference between the groups (P 〈 0.05). Conclu- sion: Mandibular second premolar extraction might be a helpful procedure on the angulations of the de- veloping mandibular third molars.
