摘要
目的 探讨鼻咽癌放射治疗后随诊发现颈部肿物时 ,用彩色多普勒超声检查对鉴别肿瘤局部复发和放射性纤维化的价值。方法 33例鼻咽癌患者放射治疗后随诊发现颈部 43个肿物 ,行彩色多普勒超声检查 ,并在其定位及鉴视下行肿物穿刺活检。记录肿物血流参数 ,用方差分析法处理各组数据。以细胞、病理学诊断为金标准 ,评价彩色多普勒超声诊断的敏感性和特异性。结果 复发组及纤维化组肿物个数分别为 30 ,13个。复发组峰值血流速度为 (2 2 .81± 7.80 )cm/s,阻力指数为 0 .5 3± 0 .0 9,血流分级Ⅲ~Ⅳ级 ;纤维化组的为 (11.18± 5 .6 1)cm/s,0 .71± 0 .2 2 ,血流分级Ⅰ~Ⅱ级。 2个组比较差异有极显著意义 (P <0 .0 1)。对肿物直径 >2cm者 ,彩色多普勒超声诊断敏感性为 10 0 .0 % ,对直径≤ 2cm者为 90 .5 % ,总敏感性为 95 .4%。结论 鼻咽癌放射治疗后 ,随诊发现颈部肿物 ,在鉴别其为放射纤维化或肿瘤复发时 。
Objective To evaluate color doppler flowing imaging(CDFI) in the differential diagnosis of local tumor recurrence and postradiation fibrosis of cervical mass after radiation therapy of nasopharyngeal carcinoma.Methods Forty three cervical masses of 33 cases of nasopharyngeal carcinoma after radiation therapy were examined by CDFI, and all masses had needle biopsy under ultrasound guide for cytology and histopathology diagnosis. The blood flow parameters of mass were recorded. Variance analysis was used as statistical management for blood flow parameters differences and compared ultrasonic diagnosis with cytological and pathological diagnosis.Results Among the 43 masses, 30 were local recurrence (RT) and 13 were postradiation fibrosis (RF), the blood flow parameters of RT masses: max (22.81±7.80)?cm/s,RI 0.53±0.09,blood flow grade: Ⅲ~Ⅳ, The RF masses: max(11.18±5.61)?cm/s , RI 0.71±0.22, blood flow grade:Ⅰ~Ⅱ. The blood flow parameters of RT and RF were significantly different(P< 0.01 ). For mass diameter > 2?cm,the accuracy of ultrasonic diagnosis was 100.0%, for mass diameter ≤2?cm the sensitivity was 90.5%.The total sensitivity was 95.4%, the specificity was 86.7%. Conclusion CDFI is an important method for differential diagnosis of cervical mass after radiotherapy.
