摘要
目的对比分析瑞舒伐他汀和阿托伐他汀对冠心病患者的调脂、非调脂作用及对动脉粥样硬化的逆转作用和安全性。方法选择2008年10月—2010年4月于我院行冠状动脉造影证实为冠心病的患者150例,均进行常规治疗,应用随机数字表法分为两组:A组76例,口服瑞舒伐他汀片10 mg/次,1次/晚;B组74例,口服阿托伐他汀10 mg/次,1次/晚,疗程均为12个月。分别于治疗前、治疗后6个月及治疗后12个月时测定血脂、超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)及颈动脉内膜中层厚度(IMT),同时观察治疗期间不良事件及肝肾功能情况。结果组间比较,两组治疗前及治疗后6个月、治疗后12个月时总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、hs-CRP、Hcy水平、颈动脉IMT比较,差异均有统计学意义(P<0.05)。组内两两比较,治疗前、治疗后6、12个月时两组以上指标间差异均有统计学意义(P<0.05)。研究期间两组均未发现与药物有关的严重不良反应。结论瑞舒伐他汀与阿托伐他汀均能有效改善冠心病患者血脂水平、降低血hs-CRP及Hcy水平,从而有利于延迟或逆转动脉粥样硬化的进程,但瑞舒伐他汀的作用较阿托伐他汀的近期效果更显著。
Objective To analyze the lipid -, non - lipid - lowering effects of rosuvastatin and atorvastatin on coro- nary heart disease (CHD), reversal effect on atherosclerosis and their safety. Methods One hundred and fifty patients diag- nosed as CHD were divided randomly into groups A (n =76, oral rosuvastatin, 10 mg/time/night, 1 month, based on conven- tional therapy), B (n=74, oral atorvastatin, 10 rag/time/night, 1 month, based on conventionaltherapy). Blood lipid, hy- persensitive C reactive protein ( hs - CRP), homocysteine ( Hcy), carotid artery intima - media thickness (IMT) were deter- mined before treatment, 6, 12 months after treatment, and adverse events and liver - kidney function observed. Results There was significant difference in total cholesterol (TC), triacylglycerol (TG), low - density lipoprotein cholesterol (LDL - C ), high- density lipoprotein cholesterol (HDL- C), hs -CRP, Hcy, IMT between 2 groups before treatment and 6, 12 months after treatment (P 〈 0. 05). No severe drug - related adverse reactions were found. Conclusion Both rosuvastatin and atorvas- tatin can effectively improve blood lipid level, lower hs - CRP and Hcy to contribute to delaying or reversing atherosclerotic process, but rosuvastatin has more remarkable short -term effect than atorvastatin.
