摘要
目的探讨重力肌群伸展强化锻炼治疗伴有活动性功能障碍的强直性脊柱炎的效果。方法选择2008年4月—2011年8月我校门诊部及周边高校医院收治的伴有活动性功能障碍的强直性脊柱炎患者37例,采用简单数字表分为对照组18例和试验组19例。对照组采用传统强直性脊柱炎锻炼方法,试验组采用对姿势有影响的缩短的重力肌群进行伸展强化锻炼。观察并记录两组锻炼前后强直性脊柱炎度量指数(BASMI)、毕氏强直性脊柱炎功能指数(BASFI)及强直性脊柱炎疾病活动指数(BASDAI)。结果两组锻炼前BASMI各项、BASFI总积分、BASDAI总积分比较,差异均无统计学意义(P>0.05);锻炼后比较,除BASDAI总积分外,余差异均有统计学意义(P<0.05)。对照组锻炼前后比较,仅BASMI中枕墙距、腰椎屈曲度差异有统计学意义(P<0.05);试验组锻炼前后比较,各指标差异均有统计学意义(P<0.05)。结论针对短缩的重力肌群进行伸展强化锻炼治疗伴有活动性功能障碍的强直性脊柱炎的效果优于传统的锻炼方法,或与之相结合可能会有更好的效果。
Objective To explore the effect of gravational muscles stretching and strengthening exercise in ankylosing spondilitis patients with activity dysfunction. Methods 37 ankylosing spondilitis patients with activity dysfunction in our school clinic and the surrounding university hospital from April 2008 to August 2011 were selected and divided into control group ( 18 cases) and experimental group ( 19 cases), using simple digital table. Conventional exercise was used in the control group where- as stretching and strengthening exercise was designated to the shortened gravational muscles which affected the posture in the ex- perimental group. BASMI, BASDAI and BASFI were applied respectively to evaluate the treatment effect. Results All the inde- xes of BASMI, BASFI and BASDAI in both groups had no significant difference before exercise (P 〉 0. 05) ; After exercise, all the indexes of the BASMI and BASFI had a much more significant improvement except BASDAI ( P 〈 0. 05 ) . In the control group, only tragus to wall and lumbar side flexion had a significant improvement after the exercise ( P 〈 0. 05 ) . In the experi- mental group, all the indexes of the BASMI, BASDAI and BASFI were significantly different before and after the exercise (P 〈 0. 05) . Conclusion Compared with the conventional exercise, the method to strengthen and stretch the shortened gravational muscles seems better to improve the spinal column function, maybe more better by combine the both.
