摘要
目的:观察伊立替康加顺铂方案联合全脑适形放疗治疗SCLC脑转移的近期疗效和不良反应。方法:伊立替康60mg/m2,第1、8、15天静点,顺铂60mg/m2,分3天静点,28天为一个周期,全脑适形放疗给予DT40Gy/20次/4周,后程脑转移局部病灶加量DT10～14Gy。在至少化疗两周期及全脑放疗结束一个月后行脑增强CT或MRI检查评价疗效。结果:IP方案联合放疗治疗SCLC脑转移的有效率达70.6%,17例患者中CR 1例,PR 11例,SD 5例,无进展病例。主要的不良反应是骨髓抑制和迟发性腹泻,且以Ⅰ～Ⅱ度居多,患者耐受性好。结论:IP方案联合全脑适形放疗治疗SCLC脑转移的近期疗效肯定,不良反应可耐受。
Objective：To observe the therapeutic efficacy and adverse events of irinotecan plus cisplatin（IP） regimen combined with whole brain conformal radiotherapy on the brain metastases from SCLC.Methods：Irinotecan 60mg/m2,iv,on day1,8,15,Cisplatin,60mg/m2,iv for three days of a 28-day cycle.The whole brain radiotherapy was given DT 40Gy,20 times for 4 weeks.In the latter course the volume on the metastatic brain lesions was increased in DT 10~14Gy.The cerebral contrast-enhanced CT or MRI examination was performed to evaluate the therapeutic efficacy after at least two cycles of chemotherapy and a month when the whole brain radiotherapy was overt.Results：The effective rate of IP regimen combined with radiotherapy was 70.6%.1 case was CR,11 cases were PR,5 cases were SD and no progress cases in all 17 patients.Conclusion：IP regimen combined with the whole brain conformal radiotherapy in treatment of brain metastases of SCLC has high efficacy and adverse effects can be tolerated.
