放射性^(125)I粒子治疗晚期非小细胞肺癌的效果 被引量:2

THE EFFICACY OF RADIOACTIVE SEEDS ^(125)I IN THE TREATMENT OF ADVANCED NON-SMALL CELL LUNG CANCER

摘要 目的探讨CT引导下经皮穿刺植入125I放射性粒子治疗晚期肺癌的临床应用价值。方法对32例不适合做外照射治疗的晚期非小细胞肺癌病人,应用治疗计划系统计算剂量和布粒计划,CT定位下行病灶内125I放射性粒子植入术。植入结束后,再次进行CT扫描观察粒子分布情况,进行质量验证。观察有无并发症等发生。治疗后半年行CT等检查,观察肿瘤体积变化判定疗效,并于1、2年评价生存率。结果肿瘤半年局部控制率为93.8%(30/32),中位生存时间12个月(6~24个月),1年和2年生存率分别为59.38%(19/32)和31.25%(10/32)。穿刺过程中有3例出现气胸,1例出现心脏事件,2例出现肺出血,均经相应治疗后好转。无长期严重并发症发生。结论 CT引导下经皮穿刺125I粒子植入治疗晚期肺癌安全、损伤小、并发症轻,疗效肯定。 Objective To assess the value of CT-guided radioactive seeds lz51 implantation for advanced non-small cell lung cancer (NSCLC). Methods Thirty-two patients with advanced NSCLC unsuitable for external irradiation were offered ra dioactive seeds t2sI implantation, TPS was employed to calculate the number of seeds and their distribution. Upon completion of the implantation, a CT scan was repeated for observing the seed distribution and quality verification, and if there were complica tions. CT scan six months later to inspect the efficacy by measuring the volume change of the tumor, the survival rate was ap- praised after one and two years. Results The six-month-loeaI control rate was 93.8% (30/32), median survival was 12 months (6 24 months), and 1 and 2-year survival was 59. 38% (19/32) and 31. 25% (10/32), respectively. During the procedures, pneumothorax, heart attack and pulmonary hemorrhage occurred in three, one and two, respectively, all were improved after cor responding therapies. No long-term severe complications were found. Conclusion CT-guided implantation of ^125I seeds for ad- vanced-stage lung cancer treatment is effective with safe, minor injuries and slight complications.
