摘要
目的探讨射频热凝联合臭氧治疗椎间盘源性下腰痛(DLBP)的临床效果。方法回顾分析自2007年1月~2009年6月临床资料完整的54例椎间盘源性下腰痛采用微创射频热凝联合臭氧治疗,先射频针行纤维环多靶点射频热凝,再臭氧脉冲式注入髓核和神经根周围,采用VAS、ODI和改良MacNab标准评价治疗效果。结果手术时间23—53min。平均27min。无神经损伤、椎间隙感染等并发症。54例均获随访2年,术前VAS评分(7.25±o.83)分显著高于术后1周(4.37±0.24)分(q=29.052,P〈O.05)、术后6个月(3.494±1.02)分(q=37.929,P〈O.05)、术后1年(2.96±0.72)分(q=43.275,P〈O.05)、术后2年(3.14±0.59)分(q=41.459,P〈0.05);术前ODI评分(34.55±4.71)分显著高于术后1周(23.274±7.39)分(q=17.431,P〈0.05)、术后6个月(19.75±4.16)分(q=22.870,P〈0.05)、术后1年(18.03±2.48)分(q=25.528,P〈0.05)、术后2年(18.124±3.58)分(q=25.389,P〈0.05)。术后2年MacNab评分有效率75.9%(41/54)。结论射频热凝联合臭氧注射是治疗椎间盘源性下腰痛比较理想的微创方法。
Objective To evaluate the clinical treatment effects of discogenic low back pain with radiofrequency combined ozone. Methods The clinical data of hospitalized patients with discogenic low back pain were retrospectively analyzed from January 2007 to June 2009. In which 54 patients with complete data were treated with minimally invasive radiofrequeney combined ozone, and the use of VAS, ODI and improved MacNab criteria to evaluate the therapeutic effect. Results The operation time was 23-53 min, an average of 27 min. No complications of nerve damage, disc space infection occurred. All of 54 patients were followed up for 2 years. The preoperative VAS score (7.25 ±0.83) was significantly higher than the preoperative score of 1 week (4.37±0.24) (q =29.052, P 〈0.05), 6 months (3.49 ±1.02) (q =37.929, P 〈0.05), 1 year (2.96± 0.72)(q=43.275, P 〈0.05), and 2 years (3A4+-0.59)(q =41.459, P 〈0.05); And the preoperative ODI score (34.55±4.71)was significantly higher than preoperative score of 1 week (23.27±7.39)(q =17.431 ,P 〈0.05), 6 months (19.75±4.16)(q =22.870, P 〈0.05), 1 year (18.03±2.48)(q =25.528,P 〈0.05), and 2 years (18.12±3.58)(q =25.389,P 〈0.05). The efficacy rate of MacNab criteria at 2 year reached 75.93%(41/54). Conclusion Treatment of discogenic low back pain with radiofrequency combined ozone is the ideal minimally invasive method.
