摘要
目的了解山西省克山病病情,为防治工作提供科学依据。方法在历史重病区交口、吉县、大宁每县选择1个病区乡,每个乡选择1个650人左右的病区村,开展临床查体和心电图描记,同时采集部分居民发样检测内环境硒含量水平。结果共检查1 879人,检出克山病患者68例,总检出率3.62%;其中慢型克山病患者6例,检出率0.32%;潜在型克山病患者62例,检出率3.30%;检出心电图异常215例,异常率11.4%;克山病患者心电图异常改变以ST-T改变和完全性右束支传导阻滞为主,分别占总异常项次的22.4%和20.4%;克山病病例男女性别比为1∶1.06;儿童和成人发硒含量分别为(0.348±0.167)mg/kg和(0.271±0.064)mg/kg。结论本次调查山西省克山病检出率高于国家控制标准,但全省总体上病情基本稳定;克山病仍是严重的公共卫生问题之一,应该充分认识到防治监测工作的长期性、重要性和艰巨性。
Objective To understand epidemic situation of Keshan disease in Shanxi Province, to provide scientific basis for control and prevention. Methods In Jiaokou, Ji and Daning County, 1 township of endemic area was selected, and 1 village with 650 residents was chosen in each township. Clinical examination and electrocardiography were performed in those chosen people at each village, and samples of hair were collected at the same time to detect for concentration of seleni- um. Results Totally 1 879 residents were investigated, and 68 cases with Keshan disease were founded with the total de- tection rate of 3.62%. Out of the 68 cases, there were 6 of the chronic with the detection rate of 0.32%, 62 of the potential with the detection rate of 3.30%. 215 cases with abnormal electrocardiograms were detected, with the abnormal rate of 11.4%. Out of the cases with abnormal electrocardiogram changes, ST-T change and complete fight bundle branch block were the main shows, accounting for 22.4% and 20.4%, respectively. The contrast of male and female was 1 ： 1.06. Selenium concentration of the samples of children and adult was （0.348±0.167） and （0.271±0.064） mg/kg. Conclusions The results of this surveillance present the most serious disease status in Shanxi Province. The detection rate of Keshan disease is high than that of national control standard. But as a whole, the disease status is steady basically. Keshan disease is still one of the serious public health problems, and the works of surveillance and control are important and arduous for long-term.
