256层螺旋CT前瞻性心电门控技术在小儿复杂先天性心脏病诊断中的价值

摘要 目的探讨256层螺旋CT前瞻性心电门控技术在小儿复杂先天性心脏病(CHD)诊断中的价值。方法回顾性分析54例采用256层螺旋CT前瞻性门控技术检查的复杂CHD患儿图像,并计算有效辐射计量,患者平均年龄2岁3个月(2月~6岁4个月),所有患儿均行超声心动图检查,9例患儿行心血管造影检查,所有病例均经手术证实。结果手术证实畸形233处,256层螺旋CT敏感度为98.7%,特异度为99.9%,准确度98.5%。平均ED为(1.02土0.45)mSv。结论 256层螺旋CT前瞻性心电门控技术,有效减少辐射计量,可以清晰显示复杂CHD的病理解剖形态,特别对主动脉、肺动脉、肺静脉及侧支血管的显示具有明显优势。 Objective To study the diagnostic value of prospective-gated step-and-shoot (SAS) technique on a 256-slice Spiral CT Scanner in Complex Congenital Heart Disease. Methods Fifty-four infants (age ranged from 2months to 6 years and 4 months with mean age of 27 monthswith complicated congenital heart diseases were examined with prospective-gated step-and-shoot (SAS) technique on a 256-slice Spiral CT Scanner ,the imaging data of fifty-four infants with CCHD were retrospectively analyzed. The radiation effective dose(ED)was calculated.All of infants were examined with echocardiography(ECG), 9 of them were examined with angiocardiography. Surgical confirmation ,which was used as the reference standard, was available in all patients. Results A total of 224 cardiac deformities confirmed by operation were studied. The sensitivity, specificity, and ac curacy of 256-slice CT in detecting cardiac deformities were 98.7%, 99.9%, 98.5%. The mean ED was(1.02±0.45)mSv . Conclusion Prospective-gated step-and-shoot (SAS) technique on a 256-slice Spiral CT Scanner results in lower radiation dose. Imaging with 256-slice spiral CT is able to display the structures and malformations of infants with complex congenital heart diseases.
