血小板活化标志物CD62P,CD63与血清胱抑素C 联合检测在糖尿病肾病早期诊断的临床研究 被引量:5

摘要 目的 观察糖尿病肾病(DN)患者血小板活化标志物CD62P,CD63表达及血清胱抑素C的变化及其相关性,探讨两者在DN早期的作用.方法 选择单纯糖尿病(DM)28例,早期糖尿病肾病30例,健康对照组26例.采用流式细胞仪(FCM)检测各组CD62P,CD63的表达水平,采用胶乳增强免疫透射比浊法检测血清胱抑素C.结果 ①单纯糖尿病组的CD62P(5.56±2.31)%和CD63(7.60±2.18)%的表达水平较正常对照组(1.60±0.56)%高(P〈0.01),早期糖尿病肾病组CD62P(8.60±2.43)%,CD63(15.67±3.35)%的表达水平较单纯糖尿病组高(P〈0.01);早期糖尿病肾病组Cys-C(0.64±0.28mg/L)结果较单纯糖尿病组(0.91±0.12mg/L)高(P〈0.01);②早期糖尿病肾病组CD62P,CD63与Cys-C呈正相关(r1=0.394,r2=0.592,P〈0.05).结论 血小板活化标志物CD62P,CD63的表达水平与血清胱抑素C浓度在糖尿病早期肾损害中有着重要的临床意义,二者联合检测可作为糖尿病肾病早期诊断指标. Objective To investigate the variation and correlation between CD62P,CD63 and serum cystatin C in early diagnosis of diabetic nephropathy, and explore their effects in eraly diabetic nephropathy. Methods 28 cases with DM,30 cases with early DN and 26 normal controls were enrolled. Flow cytometric analysis(FCM) was used to measure the levels of platelet glycoprotein CD62P and CD63, and serum Cys C concentration was determined by latex enhanced immunoturbidimetric method. Results OThe levels of platelet glycoprotein CD62P(5.56±2.31) % ,CD63(7. 60±2.18) % in DM were higher than the normal control group (P 〈 0.01) the levels of platelet glycoprotein CD62P (8.60 ±2.43) %, CD63 ( 15.67 ± 3.35) % in early DN were higher than those in DN(P〈0. 01). Serum Cys C concentration in early DN were higher than those in DN(P〈0.01). (2)platelet glycoprotein CD62P, CD63 in early DN correlated positively with Cys-C(r1 = 0. 394, r2 = 0. 592,P〈0. 01). Conclusion The levels platelet activation marker CD62P,CD63 and serum Cys C detectionwas useful in early diagnosis of DN. The combined detection for CD62P,CD63 and serum Cys C can be used as early diagnostic marker of DN.
