摘要
目的探讨胆囊癌淋巴转移的临床病理危险因素。方法回顾性分析医院2010年7月～2012年2月收治的61例胆囊癌患者的临床病理资料。分析胆囊癌淋巴转移与临床病理因素之间的关系。结果 61例患者中,有淋巴转移者50例(62.3%)。单因素分析显示,患者术前血清CEA水平、肿瘤部位、肿瘤大小、组织分化程度、T分期与胆囊癌神经及脉管浸润转移有关(P≤0.05),患者的性别、年龄、是否合并胆囊结石、术前血清CA19-9水平均与胆囊癌淋巴转移无关(P>0.05)。多因素Logistic回归分析显示,肿瘤大小和T分期与胆囊癌淋巴转移相关(P<0.05)。结论肿瘤部位和T分期是影响胆囊癌淋巴转移的独立危险因素。
[Objective] To investigate the risk factors associated with the lymph node metastasis in pa- tients with gallbladder carcinoma. [Methods] The clinical data of 61 patients with gallbladder carcinoma be- tween July 2010 and February 2012 were retrospectively analyzed. The correlation between clinicopathological characteristics and the lymph node metastasis was assessed by univariate and multivariate analyses. [Results] The proportion of patients with Lymph node metastasis out of 61 patients was 62.3% （n =38）. Univariate anal- ysis showed that the Serum carcinoembryonic antigen （CEA） level, tumor location, tumor size, differentiation and Pathologic T stage were associated with the lymph node metastasis; the gender, age, pathology, differentia- tion, lymph node metastasis and serum carbohydrate antigenic determinant 19-9 （CA19-9） level before opera- tion were not associated with the lymph node metastasis （P 〉0.05）. Multivariate logistic regression analysis showed that the tumor size and the pathologic T stage were significant independent risk factors （P 〈0.05） for lymph node metastasis, with odds ratio of 1.89 and 2.14, respectively. [Conclusion] Tumor location and pathologic T stage are independent risk factors for lymph node metastasis in gallbladder carcinoma.
