摘要
目的探讨环氧合酶2(COX-2)和P-糖蛋白(P-gp)在肝细胞癌(HCC)中的表达及与临床病理特征的关系。方法应用免疫组织化学法检测116例HCC组织和癌旁组织COX-2和P-gp表达,分析其与临床病理特征的关系。结果 COX-2在HCC组织中的高表达率明显低于癌旁组织(61.2%vs.98.3%)(P<0.01),在中高分化组中的高表达率明显高于低分化组(69.5%vs.52.6%)(P<0.05),在甲胎蛋白(AFP)增高组中的高表达率明显低于AFP正常组(55.1%vs.73.7%)(P<0.05)。P-gp在HCC组织中的高表达率明显高于癌旁组织(50.0%vs.3.5%)(P<0.01),在合并有肝硬化组中的高表达率明显低于无肝硬化组(42.3%vs.65.8%)(P<0.05)。结论 COX-2在HCC组织中具有较高表达,是反应HCC生物学行为的有效指标,对HCC的发生发展可能具有一定作用,有望成为HCC基因治疗的靶点。
Objective To investigate the role of cyclooxygenase-2 (COX-2) and P-glycoprotein (P-gp) playing on the oncogenesis and development of the hepatocellular carcinoma(HCC) and thecorrelation between COX-2 and P-gp in HCC. Methods The expressions of COX-2 and P-gp in HCC tissues(group A) and tumor-adjacent tissues (group B) in 116 cases were detected withimmunohistochemistry staining. The relationship of the results and the clinical characteristics were analyzed. Results The COX-2 high expression rate was lower in group A than that in group B(61.2%vs. 98. 3%)(P〈0. 01), which was higher in the cases with well and moderately differentiated carcinoma than that in those with the poorly differentiated carcinoma(69. 5 % vs. 52.6 %)(P〈0. 05),but was lower in the cases with higher AFP levels than that in those with normal AFP levels(55.1% vs. 73. 7%)(P〈0. 05). The P-gp high expression rate was higher in group A than that in group B(50. 0% vs. 3. 5%)(P〈0. 01), which was lower in the cases with complicated hepatic cirrhosis than that in those without(42.3% vs. 65.8%)(P〈0. 05). Conclusion COX-2 is expressed highly in HCCtissues and is a valid marker reflecting the biological behavior of HCC. COX-2 may play a role in hepatocareinogenesis and is hopeful to become a potential target of gene therapy for HCC.
