空心加压螺钉固定与人工股骨头置换治疗老年股骨颈骨折的疗效对比 被引量:6

Therapeutic effects analysis of hollow screw internal fixation and femoral head arthroplasty for elderly patients with femoral neck fracture

摘要 目的比较空心加压螺钉固定与人工股骨头置换治疗老年股骨颈骨折的疗效。方法 108例老年股骨颈骨折患者根据手术方式分为两组,50例采用空心加压螺钉固定,58例采用人工股骨头置换术,比较两种方法术中出血量、手术时间、卧床时间、再手术率、髋关节功能Harris评分及术后并发症发生情况。结果两组在手术时间、术中出血量、卧床时间及再手术率方面比较,差别均有统计学意义(P<0.01);人工股骨头置换组术后髋关节功能Harris评分明显高于空心加压螺钉组,差别有高度统计学意义(P<0.01);人工股骨头置换组术后并发症发生率(8.62%)明显低于空心加压螺钉组(32%),二组比较差别有统计学意义(P<0.01)。结论在严格掌握手术适应症的条件下,人工股骨头置换术用于治疗老年股骨颈骨折明显优于空心加压螺钉内固定。 Objective To compare the clinical effects of hollow screw internal fixation and femoral head arthroplasty for elderly patients with femoral neck fracture.Methods Totally 108 elderly femoral neck fracture patients in our department were divided into two groups: 50 patients were treated with hollow screw internal fixation(hollow screw group),and 58 patients were treated with femoral head arthroplasty(femoral head arthroplasty group).The blood loss,operation time,bed time,reoperation rate,Harris hip function evaluation and complications after operation of the two groups were compared.Results There were significantly differences in terms of blood loss,operation time,bed time and reoperation rate(P0.01).The Harris score in femoral head arthroplasty group was significantly higher than that in hollow screw group(P0.01).The incidence of complications after operation in femoral head arthroplasty group was 8.62%,which was significantly lower than that of the hollow screw group(P0.01).Conclusion Femoral head arthroplasty is effective for the elderly patients with femoral neck fractures under strictly controlled conditions of the indications for surgery.
