摘要
目的探讨2型糖尿病患者发生冠心病的相关危险因素。方法选择2型糖尿病患者150例,根据是否合并冠心病分为2组,单纯2型糖尿病患者组(A组)82例和2型糖尿病合并冠心病患者组(B组)68例。另选择同期健康体检者60例作为对照组,测定各组的同型半胱氨酸、高敏C反应蛋白、凝血酶原时间、活化部分凝血酶时间、纤维蛋白原和D-二聚体。结果与对照组比较,A组和B组患者同型半胱氨酸、高敏C反应蛋白和D-二聚体差异有统计学意义(P<0.05);与A组比较,B组同型半胱氨酸、高敏C反应蛋白和D-二聚体差异有统计学意义(P<0.05)。多因素logistic回归分析显示,同型半胱氨酸、高敏C反应蛋白和D-二聚体均为冠心病的危险因素。3组凝血酶原时间、活化部分凝血酶时间、纤维蛋白原比较差异无统计学意义(P>0.05)。结论同型半胱氨酸、高敏C反应蛋白和D-二聚体均是2型糖尿病患者发生冠心病的高风险因素。
Objective To study the risk factors for coronary heart disease(CHD) in patients with type 2 diabetes mellitus(DM). Methods One hundred and fifty type 2 DM patients were divided into simple type 2 DM group(group A,n=82)and type 2 DM accompanying CHD group(group B, n=68) ,60 healthy subjects served as a control group in this study. Their serum levels of homo- cysteine(Hcy), high sensitivity C-reactive protein(CRP) and D-dimer were measured, and their fibrinogen,prothromhin time and activated partial thrombin time were recorded. Results The serum levels of Hcy,high sensitivity CRP and D-dimer were significantly higher in groups A and B than in control group and in group B than in group A(P〈0. 05). Logistic regression analysis showed that Hcy,high sensitivity CRP and D-dimer were the risk factors for CHD. No significant difference was found in prothrombin time, activated partial thrombin time and fibrinogen among the 3 groups(P〉0.05). Conclusion Hcy,high sensitivity CRP and D-dimer are the risk factors for CHD in type 2 DM patients.
