曲面断层片在下颌骨骨折确诊的临床价值

摘要 目的评估曲面断层片在下颌骨骨折确诊的临床价值。方法回顾分析71例下颌骨骨折病例下颌骨正中联合骨折22例,颏孔区骨折18例,下颌角骨折15例,髁状突骨折16例,均行螺旋CT三维重建和曲面断层片检查,进行颌面部骨折影像对比分析,并与l临床检查和术中所见为对照。结果螺旋CT三维重建和曲面断层片对下颌骨骨折的整体诊断率两者差别无统计意义(P〉0.05);在下颌骨正中、颏孔区、下颌角骨折螺旋CT三维重建和曲面断层片诊断率两者差异无统计学意义(P〉0.05);髁突骨折螺旋CT三维重建的诊断率高于曲面断层片(P〈0.05)。结论曲面断层片能准确、直观、快捷地显示下颌骨骨折部位,费用低,可为急诊手术提供合理参考,但对髁突骨折容易漏诊,有临床体征时仍应行螺旋CT三维重建。 Objective To evaluate the clinical value of dental panoramic radiograph in final diagnosis of mandibular fractures. Methods Seventy-one cases of mandibular fractures, including mandibular symphysis fracture (n=22), mental foramen region fracture (n= 18), mandibular angle fracture (n= 15 ), and condylar process fracture (n=16)underwent spiral CT three-dimensional (3D) 3D reconstruction and dental panoramic radiography, and the results were compared with those of clinical examination and operation. Results There were no significant differences in the overall diagnosis rate for the mandibular fractures and in the diagnosis rates for the fractures in the median and mental foramen regions of mandible and mandibular angle between spiral CT three-dimensional (3D) reconstruction and dental panoramic radiography (all P 〉0.05); and the diagnosis rate for the condylar process fracture by the spiral CT three-dimensional (3D) reconstruction was significantly higher than that by the dental panoramic radiography (P 〈0.05).Conclusion Dental panoramic radiograph earl accurately, intuitively and quickly displaying the mandibular fractures with lower price, dental panoramic radiography supplies emergent clinical reference for emergency treatment, however, when there are clinical signs of condylar fracture spiral CT 3D reconstruction is the better choice to avoid missed diagnosis.
