双源CT在特发性门静脉高压症中的诊断价值 被引量:2

摘要 目的探讨双源64层螺旋CT在特发性门静脉高压症动态增强扫描中的临床诊断价值及技术要领。方法回顾性分析21例扫描资料。所有患者行双源64层螺旋CT动态增强扫描,包括平扫、动脉期、门静脉期及延迟期扫描,采用容积重建、最大密度投影、多平面重建等方法行动脉、门静脉及侧支循环血管三维成像。结果 21例中18例能够满足诊断所需。其中门静脉主干清晰成像18例,显影不全3例,为门静脉海绵样变。21例侧支循环均显影良好。接受手术者共18例,病理结果与影像学诊断完全符合。结论在特发性门静脉高压症中进行双源64层螺旋CT动态增强扫描与三维重建时,严格各项技术操作要点,能够清晰立体显示门静脉属支及侧支循环,可以提高图像质量和数据的精度,对特发性门静脉高压症具有重要的临床诊断价值。 Objective To investigate the clinical application and technical essentials of 64-slice Dual-source CT(DSCT) in enhanced dynamic CT scanning for idiopathic portal hypertension. Methods Twenty-one cases with idiopathic portal hypertension were retrospectively analyzed. Dynamic contrast-enhanced scanning including unenhanced phase, arterial phase, portal venous phase, and delay phase were performed for all patients to display artery, portal venous system and compensatory circulation, followed by three-dimensional reconstruction(maximum intensity projection,volume rendering,muhiplanar reformation) of those targets. Results 18 out of 21 can at least basically meet the parameter of diagnosis requirement. Of the 21 patients, 18 showed blurfree imaging of main portal vein(MPV) while the other 3 cases showed congenital cavernous transformation of MPV. Compensatory circulation was displayed satisfactorily in 21 cases. According to imaging information, 18 cases were treated with operations. Pathological findings were completely in accordance with three-dimensional reconstruction. Conclusion If the point of the operation can be handled when dynamic contrast-enhanced scan and 3-D reconstruction of DSCT are performed for the patients with IPH,it displays portal venous system and compensatory circulation distinctly and stereoscopically and to a great extent, dynamic contrast-enhanced scan and 3-D reconstruction of DSCT has important clinical diagnostic value to the IPH diagnosis.
