摘要
目的探讨简易呼吸器应用于大气道灌洗吸痰的临床效果及可行性。方法选取90例气管插管行机械通气治疗有吸痰指征的患者并随机分为两组,45例患者采用常规吸痰、翻身及叩背等方法作为对照组,45例患者采用简易呼吸器进行大气道灌洗作为观察组。观察比较两组患者肺部听诊情况、血气分析变化及黏膜损伤发生率。结果观察组患者吸痰后痰鸣音、肺部啰音及心率均较对照组改善更为显著,观察组患者吸痰后PaO2为(125.60±10.80)mmHg,SaO2为(96.75±2.18)mmHg;对照组分别为(100.20±10.90)mmHg,SaO2为(94.08±2.25)mmHg,两组差异均有显著性(P<0.05)。观察组黏膜损伤发生率为17.78%,对照组为40.00%,两组差异有显著性(P<0.05)。结论应用简易呼吸器进行大气道灌洗吸痰,可明显提高动脉血氧分压及血氧饱和度,改善患者呼吸困难,还可降低并发症的发生,安全可行,操作简单,疗效优于传统的吸痰方法。
【Objective】 To study the simple respirator used in atmospheric way known as irrigation sucking phlegm clinical effectiveness and feasibility.【Methods】 Select 90 cases do tracheal intubation of mechanical entilation absorption nitrogen indications are the patients and randomly divided into two groups,45 patients with normal absorption nitrogen,turn over and knock back method as control group,45 patients the simple respirator conduct atmospheric way known as irrigation as research group.Study the two groups were lung auscultation situation,the blood gas analysis change and mucosa damage rate.【Results】 The observation group patients after absorption nitrogen nitrogen sound sound,lungs and heart rate than that of the control group detect rales improve more significant,the observation group patients after absorption nitrogen PaO2 for（125.6±10.8） mmHg,SaO2 for（96.75 ± 2.18）,and the control group were（100.2 ± 10.9） mmHg,SaO2 for（94.08 ± 2.25）,two groups are statistically significant difference（P 0.05）.The observation group mucosa damage rate of 17.78%,to 40.00% in the placebo group,the two groups was statistically significant difference（P 0.05）.【Conclusion】 The application of simple respirator conduct atmospheric absorption nitrogen filling way wash,which could evidently improve blood oxygen partial pressure and the blood oxygen saturation,to improve the patient breathing difficulties,but also can reduce the incidence of complications,safe and feasible,simple operation,the effect of nuptake is better than the traditional method.
